摘　　要：目的 分析雷珠单抗对严重增生性糖尿病视网膜病变(PDR)患者M1巨噬细胞促炎细胞因子表达影响。方法 选取我院眼科2020年1月—2022年1月接诊的82例严重PDR患者展开回顾性研究,根据治疗方法的不同将其分为对照组和观察组,每组41例。对照组给予玻璃体切割术(PPV)治疗,观察组在对照组基础上于术前1周给予0.5 mg雷珠单抗玻璃体腔注射。比较两组术中新生血管出血次数、手术时间、临床疗效、黄斑中心凹厚度(CMT)、最佳矫正视力(BCVA)、血清血管内皮生长因子(VEGF)、M1巨噬细胞促炎细胞因子表达、并发症总发生率。结果 观察组术中新生血管出血次数低于对照组(P<0.05),手术时间短于对照组(P<0.05)。观察组临床总有效率(92.68%)高于对照组(65.85%)(P<0.05)。观察组治疗后CMT、血清VEGF均低于对照组(P<0.05),BCVA高于对照组(P<0.05)。观察组治疗后血清TNF-α、IL-6、IL-23水平均低于对照组(P<0.05)。观察组并发症总发生率(2.44%)低于对照组(19.51%)(P<0.05)。结论 雷珠单抗可有效缩短严重PDR患者PPV手术时间,减少术中新生血管出血次数,改善视力,抑制M1巨噬细胞促炎细胞因子表达,降低黄斑中心凹厚度及并发症发生率,临床疗效显著。Objective To analyze the effects of Leizumab on the expression of proinflammatory cytokines in M1 macrophages in patients with severe proliferative diabetic retinopathy(PDR).Methods A retrospective study was conducted on 82 patients with severe PDR treated in the ophthalmology department of our hospital from January 2020 to January 2022,and they were divided into two groups according to different treatment methods,with 41 cases in each group.The control group was given vitrectomy(PPV)treatment,and the observation group was given 0.5mg intravitreal injection of leizumab one week before surgery on the basis of the control group.The number of neovascularization bleeding,operation time,clinical efficacy,macular fovea thickness(CMT),best corrected visual acuity(BCVA),serum vascular endothelial growth factor(VEGF),M1 macrophage proinflammatory cytokine expression,and total incidence of complications were compared between the two groups.Results The number of neovascularization bleeding in observation group was lower than that in control group(P<0.05),and the operation time in observation group was shorter than that in control group(P<0.05).The total effective rate of observation group(92.68%)was higher than that of control group(65.85%)(P<0.05).After treatment,CMT and serum VEGF in the observation group were lower than those in the control group(P<0.05),and BCVA in the observation group was higher than that in the control group(P<0.05).After treatment,the serum levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-23(IL-23)in observation group were lower than those in control group(P<0.05).The total complication rate of observation group(2.44%)was lower than that of control group(19.51%)(P<0.05).Conclusion Leizumab can effectively shorten the operation time of PPV in patients with severe PDR,reduce the number of neovascular bleeding,improve visual acuity,inhibit the expression of M1-macrophage proinflammatory cytokines,reduce the thickness of macular fovea and the incidence of complications,with
