未成熟网织红细胞指数检测在帕罗西汀药物治疗过程中的应用探讨

摘　　要：目的为明确抑郁症患者在服用帕罗西汀药物治疗过程中,对红细胞相关参数水平的影响。方法回顾性分析该院2023年1月到2023年12月收治的110例服用帕罗西汀药物治疗的抑郁症患者。比较治疗前和治疗5周后患者红细胞相关参数:红细胞(RBC)、血红蛋白(HGB)、平均红细胞体积(MCV)、平均红细胞血红蛋白量(MCH)、平均红细胞血红蛋白水平(MCHC)、网织红细胞数(RET)、网织红细胞%(RET%)、未成熟网织红细胞指数(IRF)的水平。结果RBC、MCV、MCH、MCHC、RET、RET%,上述6个指标在帕罗西汀治疗5周后变化差异无统计学意义(P>0.05),HGB在帕罗西汀治疗5周后降低,差异有统计学意义(P=0.047),IRF在帕罗西汀治疗5周后显著升高,差异有统计学意义(P=0.00)。结论红细胞相关参数指标检测,特别是HGB和IRF指标检测,能在抑郁症患者服用帕罗西汀治疗过程中早期发现药物对造血系统的影响及损害,并及时进行干预,从而可尽量避免因服用帕罗西汀等抗抑郁药物的患者造成贫血等伤害,减少医患纠纷及医疗事故的发生,有效提高医疗安全性和治疗效果。Objective To clarify the effects on the levels of erythrocyte-related parameters in depressed patients taking paroxetine drug therapy.Methods 110 patients with depression taking paroxetine drug therapy admitted to our hospital from January 2023 to December 2023 were retrospectively analyzed.The levels of the patients′erythrocyte-related parameters:red blood cells(RBC),hemoglobin(HGB),mean corpuscular volume(MCV),mean corpuscular hemoglobin volume(MCH),mean corpuscular hemoglobin concentration(MCHC),reticulocyte count(RET),reticulocyte%(RET%),immature reticulocyte fraction(IRF)were compared between the patients before the treatment and after five weeks of treatment.Results RBC,MCV,MCH,MCHC,RET,RET%,the changes of the above six indexes were not statistically different after five weeks of paroxetine treatment(P>0.05),HGB was reduced after five weeks of paroxetine treatment with a statistically significant difference(P=0.047),and IRF was significantly increased after five weeks of paroxetine treatment with a statistically significant difference(P=0.00).Conclusion erythrocyte-related parameter index testing,especially HGB and IRF index testing,can detect the influence and damage of drugs on the hematopoietic system at an early stage in the process of paroxetine treatment in depressed patients and intervene in time,thus can try to avoid the harm caused by taking antidepressants such as paroxetine and other antidepressants such as anemia,reduce the disputes between doctors and patients and avoid medical accidents,and improve the safety of medical care and the therapeutic effect.
