德尔菲法分析构建老年冠心病出院准备度评价指标体系研究

摘　　要：目的构建老年冠心病出院准备度评价指标,为实施老年冠心病患者出院准备服务及评价其出院准备程度提供依据。方法于2021年1月至2023年4月选取100例老年冠心病患者,通过回顾文献与理论研究方法初步构建老年冠心病出院准备度指标评价体系雏形,选择全国数十家三甲医院心内科25名专家,采用德尔菲法进行专家函询咨询,并评价专家的权威性、积极性及意见的协调性,通过层次分析法确定各指标权重,确立老年冠心病出院准备度评价指标体系。结果25名专家的平均年龄在43岁左右,平均在岗时间(29.15±3.74)年,其中在岗8~10年有6名,10年以上19名;发放两轮专家调查问卷后的有效回收率分别为100.00%、88.00%;68.00%的专家提出建设性意见;两轮专家咨询的Cr为0.934;两轮肯德尔和谐系数分别为0.171和0.110;两轮函询全部指标重要性及满分比均数在4.11~4.99,变异系数0.00~0.21,通过两轮函询中最终确定冠心病患者出院准备度的46个指标,其中一级指标有,二级指标39个。结论本研究基于德尔菲法分析构建了一套内容覆盖全面、权重设置科学、客观的老年冠心病出院准备度评价指标体系,为进一步完善老年冠心病患者的出院准备工作提供理论和实践依据。Objective To construct the evaluation index of elderly coronary heart disease discharge readiness and provide a basis for the implementation of discharge preparation service for elderly CHD patients and the evaluation of their discharge readiness.Methods January 2021 to April 2023,100 cases of elderly patients with coronary heart disease,by reviewing the literature and theoretical research method preliminary build elderly coronary heart disease discharge readiness index evaluation system,select dozens of 3 armour hospital cardiology 25 experts,2 round of expert enquiry consultation,and evaluation of expert authority,the enthusiasm and coordination,through the hierarchy analysis method to determine the index weight,establish elderly coronary heart disease discharge readiness evaluation index system.Results The average age of the 25 experts was(43.12±1.38)years;Working years ranged from 8 to 30 years,Mean(29.15±3.74)years,Six of them have worked for 8 to 10 years,from 10 to 20 years,11th place in 20 years;The effective recovery rate of the two rounds of expert questionnaires was 100.00%and 88.00%;Another 68.00%experts put forward constructive suggestions;Cr for 2 rounds of expert consultation was 0.934;The 2-round Kendall harmony coefficient was 0.171 and 0.110;The importance of all indicators and the full score ratio of the 2 rounds of letter inquiry are 4.11 to 4.99,The coefficient of variation varies between 0.00 to 0.21,The final constructed evaluation index of discharge readiness for elderly CAD patients included 7 first-level indicators and 39 second-level indicators.Conclusion Based on the Delphi analysis,this study constructed a comprehensive,scientific and objective evaluation index system of elderly CHD,which provides theoretical and practical basis for further improving the discharge preparation of elderly patients with CHD.
