摘　　要：D-二聚体作为全关节置换术后静脉血栓栓塞及假体周围关节感染的诊断生物标志物,其诊断作用及其阈值一直存在争论。诊断静脉血栓栓塞中D-二聚体专注于在术后期间建立新的阈值;而在诊断假体周围关节感染中,与传统生物标志物红细胞沉降率(ESR)和C反应蛋白(CRP)相比,D-二聚体在全关节置换术后患者中高表达,是用于假体周围关节感染及预后评估的最优生物标志物,具有明显的价值。本文主要对D-二聚体在静脉血栓栓塞及假体周围关节感染的诊断地位和现状展开综述,以探讨其在全关节置换术中的临床应用价值。D-dimer is a diagnostic biomarker for venous thromboembolism and periprosthetic joint infection after total joint replacement,and its diagnostic role and threshold have been debated.Studies have found that D-dimer in the diagnosis of VTE focuses on establishing a new threshold during the postoperative period;In the diagnosis of periprosthesis joint infection,studies have shown that compared with traditional biomarkers of erythrocyte sedimentation rate(ESR)and c-reactive protein(CRP),D-dimer is highly expressed in patients after total joint replacement,which is the best biomarker for periprosthesis joint infection and prognosis assessment,and has obvious value.In this paper,the diagnostic status and status of D-dimer in venous thromboembolism and periprosthetic joint infection were reviewed,and the clinical application value of D-dimer in total joint replacement was discussed.
