蟾附脐贴治疗阿片类药物相关性便秘的真实世界研究

摘　　要：目的探讨真实世界中蟾附脐贴对阿片类药物相关性便秘(opioid-induced constipation,OIC)的治疗效果。方法2020-01/2022-12月作者医院收治的OIC患者60例,以随机数字表法分为治疗组(n=30)和对照组(n=30)。对照组维持现有治疗,应用乳果糖口服溶液(30 ml每次,每天1次)通便;治疗组在对照组治疗基础上应用蟾附脐贴外敷神阙,两组均治疗7天。比较两组患者治疗前后的肠功能指数(bowel function index,BFI)量表及胃肠疾病中医证候评分表积分变化。并收集治疗过程中发生的不良反应。治疗结束7天后进行随访,重新观察相关指标,判断停药后持续效应。结果治疗7天后,治疗组患者的排便难易程度、排便不尽感、便秘总体评价得分均低于对照组(P均<0.05),治疗组胃肠疾病中医证候总积分低于对照组(P<0.05)。根据BFI症状评分,治疗7天时治疗组总有效率46.67%高于对照组的3.33%(P<0.05)。两组患者均未见明显治疗相关不良反应。停药后随访7天,治疗组便秘总体评价及胃肠疾病中医证候总积分仍优于对照组(P<0.05)。结论蟾附脐贴可以改善OIC患者的症状,在真实世界研究中取得了较好的治疗效果,值得进一步研究和推广。Objective To investigate the therapeutic effect of Chanfu navel paste on opioid-induced constipation(OIC)in the real world.Methods 60 patients with OIC admitted to the author′s hospital from January 2020 to December 2022 were randomly divided into treatment group(n=30)and control group(n=30)by random number table method.The control group maintained the existing treatment and used lactulose oral solution(30 ml per time,once a day)for defecation;on the basis of the treatment of the control group,the treatment group was applied Chanfu navel paste to Shenque,and both groups were treated for 7 days.The changes of bowel function index(BFI)scale and traditional Chinese Medicine(TCM)Syndrome Scale for Gastrointestinal Diseases were compared between the two groups before treatment and at the end of treatment,the adverse reactions during the treatment were also collected.Follow-up was performed 7 days after the end of treatment,and relevant indicators were re-observed to determine the lasting effect after discontinuation.Results After 7 days of treatment,the defecation difficulty feeling of endless defecation and the overall evaluation score of constipation in the treatment group were lower than those in the control group(all P<0.05),and the total TCM syndrome score in the treatment group was lower than that in the control group(P<0.05).According to BFI symptom score,the total effective rate of the treatment group was 46.67%higher than 3.33%of the control group(P<0.05)after 7 days of treatment.No significant treatment-related adverse reactions were observed in both groups.After 7 days of follow-up,the overall evaluation of constipation and the total score of TCM syndromes of gastrointes tinal diseases in the treatment group were still better than those in the control group(P<0.05).Conclusion Chanfu navel paste can improve the symptoms of patients with OIC,and has achieved good therapeutic effect in real world research,which is worthy of further research and popularization.
