摘  要:目的 基于代谢组学研究痰湿体质单纯性超重/肥胖患者的生物标志物,探索痰湿体质与单纯性超重/肥胖之间的关联。方法 收集31例痰湿体质单纯性超重/肥胖患者(O-DM组),22名健康人(HG组)的血液样本,使用高效液相色谱-质谱联用(LC-MS)技术,利用MSDIAL软件对数据进行峰提取和代谢物鉴定,采用主成分分析(PCA)及正交偏小二乘法-判别分析法(OPLS-DA)建模,对样本进行分析。筛选出差异代谢物后,利用京都基因和基因组百科全书(KEGG)数据库、人类代谢组数据库(HMDB)数据库等进行标志物通路等的鉴定。分析方式包括单变量统计分析、层次聚类分析及通路富集分析。结果 O-DM组与HG组鉴定出470个差异代谢物,将470个差异代谢物通过本体(Ontology)分类发现主要归属于氨基酸类和衍生物、多肽类、羧酸类,其中已知代谢物中占比最多的是氨基酸类和衍生物,在氨基酸代谢方面发现O-DM组中丙氨酸、谷氨酸水平较HG组高,而甘氨酸、丝氨酸、天冬氨酸水平较HG组低。通过KEGG通路富集分析,O-DM组差异代谢物主要涉及柠檬酸(TCA)循环、丙氨酸/天冬氨酸/谷氨酸代谢及甘氨酸/丝氨酸/苏氨酸代谢等代谢通路。结论 痰湿体质单纯性超重/肥胖患者与健康人之间的差异代谢物主要有氨基酸、多肽及羧酸类等,这些差异代谢物涉及到的主要通路包括丙氨酸/天冬氨酸/谷氨酸代谢,甘氨酸/丝氨酸/苏氨酸代谢通路及TCA循环。Objective To study biomarkers in simple overweight/obesity patients with phlegmdampness constitution based on metabolomics,and to explore the association between phlegmdampness constitution of traditional Chinese medicine and simple overweight/obesity in Western medicine.Methods Blood samples were collected from 31 obese/overweight patients with phlegm-dampness constitution(O-DM group)and 22 healthy individuals(HG group),and the data were analyzed using highperformance liquid chromatography-mass spectrometry(LC-MS)technology and MSDIAL Software for peak extraction and metabolite identification.Principal component analysis(PCA)and orthogonal partial least squares discriminant analysis(OPLS-DA)were used to analyze the data from a holistic perspective.After the differential metabolites were screened,the Kyoto Encyclopedia of Genes(KEGG)database and Human Metabolome Database(HMDB)were used to identify the marker pathways.The analyses methods included univariate statistical analysis, hierarchical cluster analysis, and pathway enrichment analysis.Results A total of 470 differential metabolites were identified in the O-DM group and HG group, and 470differential metabolites were mainly classified as amino acids and derivatives, peptides, and carboxylic acidsthrough Ontology, among which amino acids and derivatives accounted for the largest proportion of knownmetabolites, and the levels of alanine and glutamate in the O-DM group were higher than those in the HG group.The levels of glycine, serine, and aspartic acid were lower than those in the HG group. KEGG pathway enrichmentanalysis showed that differential metabolites in the O-DM group were mainly involved in alanine/aspartic acid/glutamic acid metabolism, glycine/serine/threonine metabolism and tricarboxylic acid(TCA) cycle metabolismpathway. Conclusions The differential metabolites between the simple overweight/obesity patients with phlegmdampconstitution and healthy individuals mainly included amino acids, polypeptides, and carboxylic acids.
