清热生肌法治疗化脓性汗腺炎原位中厚皮片移植术失败1例  

Clearing Heat and Generating Muscle Method in the Treatment of Suppurative Hidradenitis after Failure of Medium Thick Skin Graft for One Case

摘　　要：天津中医药大学第一附属医院2021年11月10日收治1例化脓性汗腺炎(HS)(HurleyⅢ期)患者。在内科治疗的基础上,采用外科手术以彻底清除病灶。因患者病变区窦道、瘢痕形成,致原位中厚皮片移植术失败,故选择更换为中医外治法。在保留原位中厚皮片的基础上,外用京万红及复方黄柏液以清热生肌。换药时,发现原位中厚皮片的存在可以保护新鲜创面;外用京万红可去腐生肌、修复创面,促进肉芽生长;间隔使用复方黄柏液湿敷以清热解毒燥湿,对于抑制肉芽过度增生有明显效果,同时减少创面分泌物、减轻创周红肿,取得明显治疗效果。术后随访局部情况改善良好。On November 10,2021,First Teaching Hospital of Tianjin University of Traditional Chinese Medicine admitted a case of hidradenitis suppurative(HS)(Hurley StageⅢ).On the basis of medical treatment,surgical operation was used to completely remove the lesion.Due to the formation of sinuses and scars in the diseased area of the patient,the transplantation of medium and thick skin in situ failed,so the external treatment of traditional Chinese medicine was chosen.On the basis of preserving the original medium thick skin,Jingwanhong and compound Huangbai liquid were used externally to clear heat and promote muscle development.During dressing change,it was found that the presence of mid-thick skin in situ could protect the fresh wound.The external application of Jingwanhong can remove the saprophytic muscle,repair the wound and promote the growth of granulation.Using compound Huangbai liquid wet compress at intervals to clear heat,detoxify and dry dampness has obvious effect on inhibiting the hyperplasia of granulation,reducing wound secretion and reducing the redness and swelling around the wound,and achieving obvious therapeutic effect.Postoperative follow-up showed good local improvement.
