归脾汤治疗老年全髋关节置换术后血虚发热临床观察

摘　　要：目的观察归脾汤治疗全髋关节置换术(THA)后血虚发热的临床效果。方法选取2021年1月—2022年9月山东省中医院接受治疗的老年THA后血虚发热患者100例,随机分成治疗组与对照组,各50例。对照组行常规物理降温治疗,治疗组在此基础上加用归脾汤治疗,观察两组临床疗效、不良反应情况及治疗前后体温、红细胞比容(HCT)与血红蛋白(HGB)监测值。结果治疗组总有效率为96.00%(48/50),高于对照组的84.00%(42/50)(P<0.05)。两组患者术前、术后第1天、术后第3天HGB、HCT数值比较,差异无统计学意义(P>0.05);术后第5天,治疗组HGB、HCT优于对照组(P<0.05)。结论归脾汤治疗老年THA后血虚发热患者,临床效果较好,优于单纯常规降温治疗。Objective To observe the clinical effect of Guipi decoction on blood deficiency fever after operation of total hip arthroplasty(THA)of the elderly patient.Methods A total of 100 elderly patients with blood deficiency and fever after THA who received treatment in Shandong Hospital of Traditional Chinese Medicine from January 2021 to September 2022 were selected and randomly divided into treatment group and control group,with 50 cases in each group.The control group received routine physical cooling treatment,and the treatment group was treated with Guipi decoction on the basis of the control group.The clinical efficacy,adverse reactions,temperature,erythrocyte specific volume(HCT)and hemoglobin(HGB)monitoring values of the two groups were observed.Results The total effective rate of the treatment group was 96.00%(48/50),which was higher than that of the control group(84.00%,42/50)(P<0.05).There was no significant difference in HGB and HCT values between the two groups before surgery,the first day and the third day after surgery(P>0.05).On the 5th day after surgery,HGB and HCT in the treatment group were better than those in the control group(P<0.05).Conclusion Guipi decoction has a good clinical effect in treating blood deficiency fever after operation of THA of the elderly patient,and its effect is better than that of simple routine cooling treatment.
