身痛逐瘀汤联合脊神经后支阻滞治疗椎体成形术后远节腰背痛临床研究

摘　　要：目的分析身痛逐瘀汤联合脊神经后支阻滞治疗椎体成形术后远节腰背痛的临床效果。方法选取2022年1月—2023年1月在南昌市洪都中医院接受治疗的120例骨质疏松性胸腰椎骨折患者作为研究对象,应用随机数字表法分为对照组、试验1组和试验2组,每组40例。对照组行椎体成形术治疗,试验1组在对照组基础上给予脊神经后支阻滞治疗,试验2组在试验1组基础上给予身痛逐瘀汤治疗,比较三组椎体成形术后远节腰背痛情况和日本骨科协会评估治疗分数(JOA评分)。结果三组术前1 d和术后1 d视觉模拟量表(VAS)评分和JOA评分差异无统计学意义(P>0.05);比较三组术后7 d、4周和8周的VAS评分和JOA评分发现,试验2组VAS评分低于试验1组和对照组,JOA评分高于试验1组和对照组。试验1组的VAS评分低于对照组,JOA评分高于对照组,差异均有统计学意义(P<0.05)。结论身痛逐瘀汤联合脊神经后支阻滞治疗椎体成形术后远节腰背痛的临床效果良好,可有效减轻患者术后疼痛程度,促进患者术后快速康复,减轻患者腰椎功能障碍。Objective To analyze the clinical effect of Shentong Zhuyu decoction combined with posterior branch block of spinal nerve in the treatment of distal low back pain after vertebroplasty.Methods A total of 120 patients with osteoporotic thoracolumbar fracture who received treatment in Hongdu Hospital of Traditional Chinese Medicine from January 2022 to January 2023 were included as research objects and divided into control group,experimental group Ⅰ and trial group Ⅱ by random number table method,with 40 cases in each group.The control group received vertebroplasty,experimental group Ⅰ received posterior spinal nerve block on the basis of the control group,and experimental group Ⅱ received Shentong Zhuyu decoction on the basis of experimental group Ⅰ.The postoperative distal low back pain of the three groups and the Japanese Orthopaedic Association score treatment(JOA score)was compared.Results There were no significant differences in visual analogue scale(VAS)and JOA scores between the three groups(P>0.05).Comparison of the VAS scores and JOA scores of the three groups at 7 d,4 weeks and 8 weeks postoperatively revealed that the VAS scores of the experimental Ⅱ group were lower than those of the experimental Ⅰ group and the control group,and the JOA scores were higher than those of the experimental Ⅰ group and the control group.The VAS score of experimental group Ⅰ was lower than that of the control group,and the JOA score was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Shentong Zhuyu Decoction combined with posterior branch block of spinal nerve has good clinical effect in the treatment of distal low back pain after vertebroplasty,which can effectively reduce the degree of postoperative pain,promote the rapid recovery after surgery,and alleviate the lumbar dysfunction.
