开郁清热方联合苯溴马隆片治疗2型糖尿病合并高尿酸血症肝胃郁热证

摘　　要：目的探讨开郁清热方联合苯溴马隆片治疗2型糖尿病合并高尿酸血症肝胃郁热证临床效果。方法选择80例2型糖尿病合并高尿酸血症肝胃郁热证患者按随机数表法分成对照组与观察组,各40例。对照组在健康宣教、降糖治疗等基础上给予苯溴马隆片口服治疗,观察组在对照组治疗基础上联合开郁清热方口服治疗,2组均治疗8周。比较2组治疗前后中医证候积分,血糖指标、血脂指标总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG),血尿酸(UA)水平,临床疗效及不良反应发生情况。结果2组治疗后脘腹痞满、形体肥胖、胸胁胀闷、心烦易怒、口干口苦、便干积分,空腹血糖、餐后2 h血糖、糖化血红蛋白(HbAlc)、胰岛素抵抗指数(HOMA-IR)水平,TC、LDL-C、TG、UA水平均明显降低(均P<0.05),且观察组低于对照组(均P<0.05);观察组总有效率(92.5%,37/40)高于对照组(65.0%,26/40)(P<0.05);2组治疗期间均未发生药物不良反应。结论开郁清热方联合西医常规治疗能够有效降低2型糖尿病合并高尿酸血症肝胃郁热证血糖、血脂及血尿酸水平,可调节患者糖脂代谢紊乱状态,促进临床症状缓解。Objective To explore the clinical effect of Kaiyu Qingre Formula combined with benzbromarone tablets in the treatment of type 2 diabetes with hyperuricemia with syndrome of heat stagnation in liver and stomach.Methods 80 patients with type 2 diabetes with hyperuricemia with syndrome of heat stagnation in liver and stomach were selected and divided into the control group and the observation group,with 40 cases in each group.The control group received oral treatment with benzbromarone tablets on the basis of health education and hypoglycemic treatment,while the observation group received oral treatment with Kaiyu Qingre Formula on the basis of the treatment of the control group.Both groups were treated for 8 weeks.The traditional Chinese medicine(TCM)syndrome scores,blood glucose indicators,blood lipid indicators[total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),triglycerides(TG)],and blood uric acid(UA)levels were compared between two groups.The clinical efficacy and incidence of adverse drug reactions were also compared.Results After treatment,the scores of amassment disease of epigastrium and abdomen and distention and fullness,obesity,fullness and discomfort in chest and hypochondrium,irritability,dry mouth and bitter taste,dry stool,and the levels of fasting blood glucose,2-hour postprandial blood glucose,glycated heamoglobin A1c(HbAlc),HOMA-IR,TC,LDL-C,TG and UA were significantly reduced in both groups(P<0.05),and the observation group was lower than the control group(P<0.05);The total effective rate in the observation group(92.5%,37/40)was higher than that in the control group(65.0%,26/40)(P<0.05);No significant adverse drug reactions occurred during the treatment period in both groups.Conclusion Kaiyu Qingre Formula combined with conventional Western medicine treatment can effectively reduce the levels of blood sugar,blood lipids and UC in type 2 diabetes patients with hyperuricemia with syndrome of heat stagnation in liver and stomach,regulate the disorder of glucose and lipid metabolism in
