紧密型县域医共体对村医队伍稳定性的影响研究

摘　　要：目的:探究紧密型县域医共体对乡村医生队伍稳定性的影响及其因素,为后续提高乡村医生稳定性提供参考。方法:采用解释性序列设计,先进行乡村医生稳定性的问卷调查,再针对稳定性评分最差和最好的医生进行定性访谈,以补充定量分析结果。结果:共回收问卷718份,定量分析结果显示是否实施紧密型县域医共体对乡村医生队伍的稳定性没有影响(P=0.19);进一步对15名乡村医生访谈,发现可能原因是村医角色转换困难、信息化适应困难、社会地位不高、职业发展路径受限和政策驱动不足等。结论:后期要进一步提高村医收入和待遇,解决村医身份问题和允许乡村两级合理流动执业,给村医多提供外出培训机会,培养其适应信息化的能力,加大当前政策对村医的支持力度,监督政策落实,增强村医的稳定性。Objective To explore the impact of compact county medical communities on the stability of village doctor teams and its factors,and provide references for improving the stability of village doctors in the future.Methods Explanatory sequential design was employed.First,a questionnaire survey was conducted on the stability of village doctors,followed by qualitative inter-views with doctors with the lowest and highest stability scores to complement the quantitative analysis.Results A total of 718 ques-tionnaires were collected.The quantitative analysis results showed that the implementation of compact county medical communities had no significant impact on the stability of village doctor teams(P=0.19).Further interviews with 15 village doctors revealed that the possible reasons for the lack of impact of compact county medical communities on village doctors included dfficulties in role transition,challenges in adapting to informatization,low social status,limited career development paths,and insufficient poli-cy support.Conclusion It is necessary to further improve the income and remuneration of village doctors,solve the problem of vil-lage doctors'identity,allow reasonable mobile practice at the rural level,provide more training opportunities for village doctors,cultivate their ability to adapt to informationization,increase the support of current policies for village doctors,supervise the imple-mentation of policies and enhance the stability of village doctors.
