托法替布治疗原发性皮肤淀粉样变一例并文献复习

摘　　要：原发性皮肤淀粉样变是一种慢性瘙痒性疾病,目前缺乏标准的治疗方案,传统疗法如抗组胺药、沙利度胺和局部糖皮质激素等疗效欠佳。有研究认为纠正瘙痒-搔抓-瘙痒的恶性循环是治疗原发性皮肤淀粉样变的关键,托法替布是一种口服泛JAK抑制剂,可显著降低特应性皮炎小鼠模型的抓挠行为和IL-4和IL-31的表达,本文报道一例托法替布治疗原发性皮肤淀粉样变并复习相关文献。Primary cutaneous amyloidosis is a chronic itching disease.At present,there is a lack of standard treatment plans for this disease,and traditional therapies such as antihistamines,thalidomide,and local glucocorticoids have poor therapeutic effects.Studies have suggested that correcting the itch-scratch-itch cycle is the key to treating primary cutaneous amyloidosis.Tofacitinib is an oral JAK inhibitor,it can significantly reduce the scratching behavior and the expression of IL-4 and IL-31 in a mouse model of atopic dermatitis.A case of primary skin amyloidosis treated with tofacitinib is reported and the related articles are reviewed.
