摘  要:目的:探讨牛肺表面活性剂联合持续气道正压通气(CPAP)对新生儿呼吸窘迫综合征(NRDS)患儿肺通气和氧合功能的影响。方法:选择NRDS患儿93例,随机分为试验组(n=46)和常规组(n=47)。常规组予CPAP治疗,试验组在此基础上予注射用牛肺表面活性剂治疗,治疗7 d对比两组疗效、肺通气和氧合功能、炎症反应。结果:治疗后,试验组总有效率、达峰时间比、第1秒用力呼气容积、氧合指数高于常规组;气道阻力、呼吸指数、白介素-6及降钙素原水平低于常规组(P<0.05)。结论:牛肺表面活性剂联合CPAP治疗NRDS疗效确切,可减轻炎症反应,提升患儿氧合功能,改善肺通气。Objective:To investigate the effect of bovine-derived pulmonary surfactant combined with continuous positive airway pressure(CPAP)on pulmonary ventilation and oxygenation in children with neonatal respiratory distress syndrome(NRDS).Methods:Ninety-three children with NRDS were randomly divided into an experimental group(n=46)and a conventional group(n=47)based on a random number table method.The conventional group was treated with CPAP,while the experimental group was additionally treated with bovine lung surfactant for injection on this basis.The efficacy,lung ventilation and oxygenation function and inflammatory response were compared between the two groups 7 days after treatment.Results:The total effective rate,the time to peak tidal expiratory flow as a proportion of expiratory time,the force expiratory volume in 1 second and the oxygenation index were higher,while the airway resistance,the respiratory index,the levels of interleukin-6 and procalcitonin were lower in the experimental group than the conventional group(P<0.05).Conclusion:The combination of bovine lung surfactant and CPAP for the treatment of NRDS has a definite therapeutic effect,which can reduce inflammatory reactions and enhance oxygenation and pulmonary ventilation in children.
