清热消痈止痛方贴敷联合光子治疗预防剖宫产术后感染的临床效果及安全性分析

摘　　要：目的 探讨清热消痈止痛方贴敷联合光子治疗对剖宫产术后感染、切口愈合效果的影响,并分析安全性。方法 选取2022年1月至2023年7月收治的行剖宫产的患者150例,采用随机数字表法分为贴敷组、光子组、联合组各50例。贴敷组给予清热消痈止痛方贴敷治疗,光子组给予光子治疗,联合组给予清热消痈止痛方贴敷联合光子治疗。对比3组剖宫产术后感染情况、术后康复进程、术后切口愈合效果、局部微循环指标、血清炎性因子水平[超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞计数]及切口疼痛评分,记录3组不良反应发生情况。结果 3组腹部浅表切口感染及发热发生率比较差异无统计学意义(P>0.05)。联合组切口愈合时间、下床活动时间、住院时间短于贴敷组、光子组,切口愈合良好率高于贴敷组、光子组(P<0.05)。术后3 d,联合组切口局部毛细血管管径、血流速度高于贴敷组、光子组(P<0.05)。术后1、3 d,联合组hs-CRP、TNF-α、白细胞计数低于贴敷组、光子组(P<0.05)。联合组术后12 h、24 h、2 d、3 d切口疼痛评分低于贴敷组、光子组(P<0.05)。3组不良反应发生率比较差异无统计学意义(P<0.05)。结论 清热消痈止痛方贴敷联合光子治疗预防剖宫术后感染效果较好,且不增加不良反应,安全性较高。Objective To investigate the effect of application of Qingre Xiaoyong Zhitong prescription combined with photon therapy on the incidence of infection and wound healing after cesarean section(C-section),and to analyze its safety.Methods A total of 150 patients who underwent C-section from January 2022 to July 2023 were selected and divided into ap-plication group(n=50),photon therapy group(n=50)and combination group(n=50)by random number table method.The application group was treated with Qingre Xiaoyong Zhitong prescription,the photon therapy group was treated with photon therapy,and the combination group was treated with Qingre Xiaoyong Zhitong prescription combined with photon therapy.Postoperative infection,postoperative rehabilitation,postoperative incision healing effect,local microcirculation index,serum inflammatory factor levels[hypersensitive C reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),white blood cell(WBC)count]and incision pain score of the three groups were compared.The incidence of adverse reactions in the three groups were recorded.Results There was no significant difference in the incidence of superficial abdominal incision infection and fever among the three groups(P>0.05).The wound healing time,time of off-bed activity and length of hospitalization in combination group were shorter than those in application group and photon therapy group,and the rate of good wound healing was higher than that in application group and photon group(P<0.05).At 3 d after operation,the local capillary diameter and blood flow velocity in combination group were higher than those in application group and photon therapy group(P<0.05).At 1 and 3 d after operation,hs-CRP,TNF-αand WBC counts in the combination group were lower than those in the application group and the photon therapy group(P<0.05).Incision pain scores of combination group were lower than those of application group and photon therapy group at 12 h,24 h,2 d and 3 d after operation(P<0.05).There was no significant difference in the incidence o
