魏有刚基于“奇恒之腑”论治胰腺癌经验

摘　　要：胰腺癌作为世界范围内人群健康的威胁,其恶性程度极高,寻求中医药诊治思路改善胰腺癌患者的生存预后非常迫切。魏有刚在中医药防治肿瘤领域积累了丰富的临床经验,对胰腺癌的治疗有独到见解,对胰腺的“奇恒之腑”概念初探,明确将胰腺归于“奇恒之腑”。魏有刚强调“肝主疏泄”,治宜疏肝解郁;强调“脾主运化”,治宜顾护脾阳;强调“胆汁排泄”,治宜利胆退黄;强调“大肠传化”,治宜行气导滞,并提出常用中药及方剂。本文列举验案,胰腺恶性肿瘤伴肝、多发淋巴结转移,湿盛毒结证,自拟茵梅汤利湿退黄,祛寒抗癌,随证加减,患者面色晦暗及身目黄染情况较前明显减轻,纳寐明显好转,有效缓解患者症状,提高患者生活质量。Pancreatic cancer is a threat to people’s health all over the world,and its malignant degree is extremely high.It is urgent to seek traditional Chinese medicine(TCM)diagnosis and treatment ideas to improve the survival and prognosis of pancreatic cancer patients.Wei Yougang has accumulated rich clinical experience in the field of preventing and treating tumors with TCM,and has unique views on the treatment of pancreatic cancer.He has made a preliminary study on the concept of"extraordinary fu-viscera"of the pancreas,and clearly classified the pancreas as"extraordinary fu-viscera".Wei Yougang emphasizes"liver controlling conveyance and dispersion",and the treatment should be soothing the liver and relieving depression.He emphasizes the"spleen governing transportation and transformation"and treats the spleen yang properly.He emphasizes"bile excretion"and treats it with promoting bile retention and reducing jaundice.His emphasis was placed on the transmission of large intestine,and the appropriate treatment was to promote the circulation of qi and stagnation,while proposing commonly used traditional Chinese medicine and formulas.In this paper,the proven cases are listed.Pancreatic malignant tumor with liver and multiple lymph node metastasis,with dampness and toxic accumulation,self-made Yinmei decoction can relieve dampness and jaundice,dispel cold and resist cancer.With the modification of the syndrome,the patient’s complexion and body and eyes are obviously reduced,and the insomnia is obviously improved,which effectively relieves the symptoms and improves the quality of life of patients.
