大庆油田总医院呼吸内科血流感染患者住院费用构成及其影响因素分析

摘　　要：目的通过对大庆油田总医院呼吸内科血流感染患者住院费用构成及其影响因素的研究,找出影响患者住院费用的主要因素。方法选择2021年1月—2022年12月于大庆油田总医院呼吸内科住院治疗的确诊为血流感染的全部患者的病案资料,采取描述性统计方法了解费用构成,对患者住院费用影响因素进行单因素分析和多因素分析。结果共收集有效患者资料255例,男167例,女88例,年龄71~80岁及81~90岁患者数量最多;住院时间11~20 d患者最多;在所有病例中,最多见的病原菌是大肠埃希菌,占比34%。住院费用构成主要包括治疗药物费用、综合医疗服务费及诊断费用,其中治疗药物费用为主要部分,占比45%。单因素及多因素分析提示血流感染患者住院费用的主要影响因素为住院时间、qSOFA评分、付费方式、出院转归,差异均有统计学意义(P<0.05)。结论药品费用是血流感染患者住院费用的主要组成部分,住院时间是影响患者住院费用的主要因素,减少药品的费用支出和患者住院时间是控制住院费用增长的主要方向。Objective To find out the main factors affecting hospitalization expenses by studying the composition of hospitalization expenses and its influencing factors for patients with bloodstream infections in the respiratory department of Daqing Oilfield General Hospital.Methods Medical records of all patients diagnosed with bloodstream infections who were hospitalized in the respiratory department of Daqing Oilfield General Hospital from January 2021 to December 2022 were selected.Descriptive statistical methods were used to understand the cost composition,and univariate and multivariate analyses were performed on the influencing factors of hospitalization costs.Results A total of 255 patients with effective information were collected in this study,with a male to female ratio of 167:88.The largest number of patients were aged between 71-80 and 81-90 years old;most patients were hospitalized for 11-20 days.In all cases,the most common pathogen was E.coli,accounting for 34%.The hospitalization fee structure mainly included treatment drug costs,comprehensive medical service fees,and diagnostic fees,with treatment drug costs being the main part,accounting for 45%.Univariate and multivariate analyses indicated that the main influencing factors of hospitalization expenses for patients with bloodstream infections were length of hospital stay,qSOFA score,payment method,and discharge outcomes(P<0.05).Conclusions Drug expenses are the main component of hospital costs for patients with bloodstream infections,and length of hospital stay is a major factor influencing hospital costs.Reducing drug expenditure and in-hospital time are the main directions for controlling the growth of hospitalization costs.
