托特罗定与坦索罗辛在缓解泌尿系结石术后双J管所致下尿路症状及生活质量影响的对比研究

摘　　要：目的 对比托特罗定与坦索罗辛在缓解泌尿系结石术后双J管所致下尿路症状及生活质量影响。方法 选取2021年7月—2022年12月期间在驻马店市中心医院诊治的泌尿系结石术后留置双J管患者146例,随机分为对照组(n=73,坦索罗辛治疗)和研究组(n=73,托特罗定治疗),比较两组下尿路症状、生活治疗和并发症。结果 相较于治疗前,两组治疗后4周梗阻症状、刺激症状国际前列腺症状评分(international prostate symptom score, IPSS)及IPSS总分降低(P<0.05),且相较于对照组,研究组治疗后4周梗阻症状、刺激症状IPSS及IPSS总分更低(P<0.05)。相较于治疗前,两组治疗后4周生理、环境、心理、社会关系领域评分及世界卫生组织生存质量测定量表简表(world health organization quality of life scale, WHOQOL-BREF)总分升高(P<0.05),且相较于对照组,研究组治疗后4周生理、环境、心理、社会关系领域评分及WHOQOL-BREF总分更高(P<0.05)。研究组尿路刺激、血尿、夜尿增多、发热、尿路感染发生率相较于对照组更低(P<0.05)。结论 相较于坦索罗辛,托特罗定在缓解泌尿系结石术后双J管所致下尿路症状效果更佳,且可更有效提高生活质量,降低并发症。Objective To compare the effects of tolterodine and tansulosin on alleviating lower urinary tract symptoms and quality of life caused by double J catheterization after surgery for urinary tract stones.Methods Totally 146 patients with postoperative indwelling double J catheters for urinary tract stones treated in our hospital from July 2021 to December 2022 were randomly divided into control group(n=73,treated with tansulosin)and study group(n=73,treated with tolterodine).The lower urinary tract symptoms,lifestyle treatments,and complications of the two groups were compared.Results Compared to that before treatment,the International Prostate Symptom Score(IPSS)and total IPSS scores for obstructive and irritating symptoms decreased in both groups at 4 weeks after treatment(P<0.05),and compared to the control group,the study group had lower IPSS and total IPSS scores for obstructive and irritating symptoms at 4 weeks after treatment(P<0.05).Compared to that before treatment,the scores in the fields of physiology,environment,psychology,and social relations,as well as the total score of the World Health Organization Quality of Life Scale(WHO QOL-BREF),increased in both groups at 4 weeks after treatment(P<0.05),and compared to the control group,the study group had higher scores in the fields of physiology,environment,psychology,and social relations,and the total score of WHO QOL-BREF at 4 weeks after treatment(P<0.05).The incidence of urinary tract irritation,hematuria,nocturia,fever,and urinary tract infection in the study group was lower than that in the control group(P<0.05).Conclusion Compared to tamsulosin,tolterodine has a better effect in relieving lower urinary tract symptoms caused by double J catheters after surgery for urinary tract stones,and can more effectively improve quality of life and reduce complications.
