摘 要:目的 通过比较对IgG、IgA、IgM的检测值、阳性检出率、确诊率、灵敏度、特异度等各项评价指标来对比探析透射比浊法及散射比浊法在免疫球蛋白(immunoglobin, Ig)临床检测中的应用价值。方法 回顾性分析2020年1月—2022年12月郑州人民医院收治的80例类风湿性关节炎患者(观察组)和同期80例健康体检人群(对照组)的病案资料。入院后采集制备研究对象血清样本,并采用透射比浊法、散射比浊法对IgG、IgA、IgM水平进行检测。比较两种检查方式的检出结果及效能分析。结果 观察组透射比浊法、散射比浊法检测所得IgG、IgA、IgM值均高于对照组(透射比浊法:t=21.478、20.975、31.940,散射比浊法:t=21.878、21.493、30.657,P<0.05);透射比浊法、散射比浊法联合检查观察组的IgG、IgA、IgM阳性检出率分别为100.00%、95.00%、97.50%高于单独检查,确诊率(99.38%)、灵敏度(100.00%)、阴性预测值(100.00%)高于单一指标(P<0.05)。透射比浊法和散射比浊法的IgG、IgA、IgM检测值、阳性检出率、确诊率、灵敏度、特异度、阳性预测、阴性预测差异无统计学意义(P>0.05)。结论 射比浊法和散射比浊法在IgG、IgA、IgM检测中具有一定应用价值,但散射比浊法的检出效果略优,临床可适当联合两种检查方式,以提升诊断效能,或视患者经济负担、医疗条件,选取更适宜的检测方法。Objective To explore the application value of transmission turbidimetry and scattering turbidimetry in the clinical detection of immunoglobulin(Ig)by comparing the detection value,positive detection rate,definite diagnosis rate,sensitivity and specificity of IgG,IgA and IgM.Methods The medical records of 80 patients with rheumatoid arthritis admitted to our hospital from January 2020 to December 2022(observation group)and 80 healthy people who underwent physical examination during the same period(control group)were retrospectively analyzed.After admission,serum samples of the subjects were collected and prepared,and IgG,IgA and IgM levels were detected by transmission turbidimetry and scattering turbidimetry.The detection results and efficacy analysis of the two methods were compared.Results The values of IgG,IgA and IgM in observation group were higher than those in control group by transmission turbidimetry and scattering turbidimetry(t=21.478,20.975,31.940,t=21.878,21.493,30.657,P<0.05).The positive detection rates of IgG,IgA and IgM in the observation group were 100.00%,95.00%and 97.50%,respectively,which were higher than those of single examination,and the diagnosis rate(99.38%),sensitivity(100.00%)and negative predictive value(100.00%)were higher than those of single index(P<0.05).There were no significant differences in the detection values of IgG,IgA,IgM,positive detection rate,diagnosis rate,sensitivity,specificity,positive prediction and negative prediction between transmission turbidimetry and scattering turbidimetry(P>0.05).Conclusion Radiation turbidimetry and scattering turbidimetry have certain application value in the detection of IgG,IgA and IgM, but the detection effect of scattering turbidimetry is slightly better. Clinical combination of the two methodscan be appropriate to improve the diagnostic efficiency, or according to the patient's financial condition and medical conditions, choose a more appropriate detection method.
