摘　　要：目的观察止血带对高能量损伤Pilon骨折围手术期临床疗效的影响。方法50例高能量损伤闭合性Pilon骨折患者分为试验组(术中未使用止血带,25例)和对照组(术中使用止血带,25例)。比较两组手术时间、手术视野清晰度评分、术中出血量、术后引流量、切口愈合情况以及术后小腿肿胀程度、疼痛情况和镇痛药物用量。结果与对照组相比,试验组手术时间长,手术视野清晰度评分低,术中出血量多,但术后引流量少,切口愈合不良发生率低,术后小腿肿胀程度轻,VAS疼痛评分低,镇痛药物用量少(P<0.01或P<0.05)。结论高能量损伤Pilon骨折患者术中未使用止血带可能获益更大。Objective To observe the effect of tourniquet on perioperative clinical efficacy of high-energy injured Pilon fracture.Methods Fifty patients with high-energy closed Pilon fracture were divided into two groups.The patients in group A(25 cases)did not use tourniquet during surgery,and those in group C(25 cases)used a tourniquet.The duration of operation,clarity score of operative visual field,intraoperative blood loss,postoperative drainage volume,incision healing,postoperative swelling of the lower leg,pain and analgesic dosage were compared between the two groups.Results Compared with group C,the duration of operation was longer,operative visual field clarity score was lower,intraoperative blood loss was more,while postoperative drainage volume was less,incidence of poor incision healing was lower,postoperative swelling of the lower leg was less,VAS pain score was lower,and analgesic drug dosage was less in group A(P<0.01 or P<0.05).Conclusion The patients with high-energy injured Pilon fracture may benefit more from abandoning the use of tourniquet during surgery.
