摘 要:目的分析子痫前期孕妇血清及胎盘组织中血管内皮生长因子(VEGF)、可溶性血管内皮生长因子受体1(sFlt-1)、胎盘生长因子(PLGF)水平及其临床意义。方法队列研究。抽取2021年12月至2023年12月于河南省人民医院生产的94例子痫前期患者,其中轻度子痫前期45例(轻度子痫前期组),重度子痫前期49例(重度子痫前期组);子痫前期患者均接受剖宫产。另抽取医院同期剖宫产孕妇40例作为对照组。观察3组孕妇血清及胎盘组织VEGF、sFlt-1及PLGF水平,分析各指标之间的相关性。结果单因素方差分析结果显示,3组孕妇血清、胎盘组织中VEGF、sFlt-1、PLGF表达水平比较差异有统计学意义(P<0.05);LSD-t检验结果显示,轻度子痫前期组、重度子痫前期组血清及胎盘VEGF、PLGF水平低于对照组,sFlt-1水平高于对照组(P<0.05);轻度子痫前期组VEGF、PLGF水平高于重度子痫前期组,sFlt-1水平低于重度子痫前期组(P<0.05)。Pearson相关性分析结果显示,血清VEGF、PLGF与胎盘VEGF、PLGF呈正相关(P<0.05),与sFlt-1呈负相关(P<0.05);血清sFlt-1与胎盘VEGF、PLGF呈负相关(P<0.05),与胎盘sFlt-1呈正相关(P<0.05)。结论不同严重程度子痫前期患者其血清、胎盘中VEGF、sFlt-1及PLGF水平存在明显差异,且两者之间存在密切相关性,或参与患者疾病发生与病程进展。ObjectiveTo analyze the levels of vascular endothelial growth factor(VEGF),soluble vascular endothelial growth factor receptor 1(sFlt-1)and placental growth factor(PLGF)in serum and placenta of preeclampsia pregnant women,and to investigate their clinical significance.MethodsA total of 94 patients with preeclampsia who gave birth in Henan Provincial People’s Hospital from December 2021 to December 2023 were selected for the cohort study,including 45 patients with mild preeclampsia(mild preeclampsia group)and 49 patients with severe preeclampsia(severe preeclampsia group).And they all received cesarean section.Another 40 pregnant women with cesarean section in the hospital during the same period were selected as the control group.The levels of VEGF,sFlt-1 and PLGF in serum and placental tissues of pregnant women in the three groups were observed.The correlations between each indicator were analyzed.ResultsUnivariate analysis of variance showed that the expression levels of VEGF,sFlt-1 and PLGF in serum and placental tissue of the three groups were significantly different(P<0.05).The results of LSD-t test showed that the levels of VEGF and PLGF in serum and placenta in the mild preeclampsia group and the severe preeclampsia group were significantly lower than those in the control group(P<0.05).The levels of sFlt-1 in serum and placenta in the mild preeclampsia group and the severe preeclampsia group were significantly higher than that in the control group(P<0.05).The mild preeclampsia group had higher levels of VEGF and PLGF,moreover,the mild preeclampsia group had lower sFlt-1 level compared with the severe preeclampsia group(P<0.05).Pearson correlation analysis results showed that serum VEGF and PLGF were positively correlated with VEGF and PLGF in placental tissue,and were negatively correlated with sFlt-1(P<0.05).Serum sFlt-1 was negatively correlated with VEGF and PLGF,and was positively correlated with sFlt-1 in placental tissue(P<0.05).ConclusionsThe levels of VEGF,sFlt-1 and PLGF in serum and placental ti
