血清PSA、TSGF、HSP水平对前列腺增生患者TUPKP术预后的预测价值

摘　　要：目的探讨血清前列腺特异性抗原(PSA)、肿瘤相关物质群(TSGF)、热休克蛋白(HSP)水平在前列腺增生患者尿道前列腺等离子切除术(TUPKP)术后动态变化情况,并分析三者联合检测对患者TUPKP术预后的预测价值。方法回顾性抽取2020年9月至2023年9月榆林市第一医院收治的83例前列腺增生患者为观察组,按照1∶1选例原则,抽取同期健康体检者83例为对照组。比较两组术前血清PSA、TSGF、HSP水平,依据观察组患者术后3个月预后情况将患者分为预后良好组与预后不良组,比较不同预后患者术前,术后1、3个月血清PSA、TSGF、HSP水平、国际前列腺症状评分(IPSS)并分析相关性,分析各血清指标联合检测对前列腺增生患者TUPKP术预后不良的预测价值。结果观察组血清TSGF、PSA、HSP水平高于对照组(P<0.05)。预后不良组血清TSGF、PSA、HSP水平、IPSS评分高于预后良好组(P<0.05);预后不良组与预后良好组术前血清TSGF、PSA、HSP水平、IPSS评分高于术后1、3个月,且术后1个月TSGF、PSA、HSP水平高于术后3个月,差异有统计学意义(P<0.05)。血清PSA、TSGF、HSP水平与IPSS评分呈正相关,差异有统计学意义(P<0.05);血清PSA、TSGF、HSP水平联合预测前列腺增生患者TUPKP术后发生预后不良的曲线下面积为0.902,约登指数为0.783,敏感度为93.10%,特异度为85.19%。结论前列腺增生患者TUPKP术后血清TSGF、PSA、HSP水平与治疗效果、患者TUPKP术预后密切相关,可为临床预测前列腺增生患者TUPKP术预后提供参考。ObjectiveTo investigate the dynamic changes of serum levels of prostate-specific antigen(PSA),tumor supplied group of factors(TSGF)and heat shock protein(HSP)in patients with benign prostatic hyperplasia after transurethral plasmakinetic resection of the prostate(TUPKP),and to analyze the predictive value of the combined detection of the three for the prognosis of patients after TUPKP.MethodsA total of 83 patients with benign prostatic hyperplasia treated in Yulin First Hospital from September 2020 to September 2023 were retrospectively selected as an observation group,and 83 healthy people treated in the hospital during the same period were selected as a control group according to the principle of 1∶1 case selection.The preoperative levels of serum PSA,TSGF and HSP were compared between the two groups.According to the 3-month postoperative prognosis,patients in the observation group were further divided into a good prognosis group and a poor prognosis group.The levels of serum PSA,TSGF,HSP and international prostate symptom score(IPSS)before operation,1 and 3 months after operation were compared among patients with different prognoses,and the correlation was analyzed.The predictive value of combined detection of these serum indexes for poor prognosis of patients with benign prostatic hyperplasia after TUPKP was analyzed.ResultsThe levels of serum TSGF,PSA and HSP in the observation group were significantly higher than those in the control group(P<0.05);the levels of serum TSGF,PSA,HSP and IPSS score in the poor prognosis group were significantly higher than those in the good prognosis group(P<0.05).In the good and poor prognosis groups,the preoperative levels of serum TSGF,PSA,HSP and preoperative IPSS score were higher than 1-and 3-month postoperative levels and scores(P<0.05),moreover,the 1-month postoperative indexes were higher than the 3-months postoperative indexes in the same group(P<0.05).The levels of serum PSA,TSGF and HSP were positively correlated with IPSS score(P<0.05);the area under the curve of
