血清CRP、PCT及IL-6水平对新生儿肺炎疗效的评估价值

摘　　要：目的探讨血清C反应蛋白(CRP)、降钙素原(PCT)和白细胞介素-6(IL-6)水平对新生儿肺炎疗效的评估价值。方法病例对照研究。抽取2022年1月至2023年12月郑州大学第一附属医院收治的新生儿肺炎患儿100例,依据治疗效果将其分为治疗效果良好组(80例)和治疗效果无效组(20例)。于治疗前及治疗后,采集患儿清晨空腹状态肘部静脉血检测血清CRP、PCT和IL-6水平。比较两组血清CRP、PCT、IL-6水平,统计受试者工作特征曲线下面积(AUC),分析血清CRP、PCT和IL-6表达对治疗效果的评估价值。结果治疗后,治疗效果良好组血清CRP、PCT、IL-6水平低于治疗前(P<0.05),治疗效果无效组血清CRP、PCT、IL-6水平与治疗前比较差异未见统计学意义(P>0.05)。治疗后,治疗效果良好组血清CRP、PCT、IL-6水平低于治疗效果无效组(P<0.05)。受试者工作特征曲线结果显示,治疗前CRP、PCT、IL-6联合检测评估新生儿肺炎疗效的AUC为0.904,高于三项指标单独检测的0.628、0.653、0.777。结论肺炎新生儿血清CRP、PCT、IL-6异常表达,三者可用于辅助评估新生儿肺炎的治疗效果。ObjectiveTo analyze the significance of levels of serum C-reactive protein(CRP),procalcitonin(PCT),and interleukin-6(IL-6)in evaluating the therapeutic effect on neonatal pneumonia.MethodsA total of 100 neonates with pneumonia admitted to the First Affiliated Hospital of Zhengzhou University from January 2022 to December 2023 were selected for the case control study,and they were divided into the good effect group(80 cases)and the ineffective group(20 cases)according to the treatment efficacy.Before and after treatment,the fasting elbow venous blood was collected at early morning for testing levels of serum CRP,PCT and IL-6.The levels of serum CRP,PCT,and IL-6 in the two groups were compared.And the area under the receiver operating characteristic curve(AUC)was recorded to analyze the value of expressions of serum CRP,PCT and IL-6 in evaluating treatment effect.ResultsAfter treatment,the serum levels of CRP,PCT,and IL-6 in the good effect group were lower than those before treatment(P<0.05),while the levels of serum CRP,PCT,and IL-6 in the ineffective group had no significant difference compared with preoperative levels(P>0.05).After treatment,the serum levels of CRP,PCT,and IL-6 in the good effect group were lower than those in the ineffective group(P<0.05).The results of the receiver operating characteristic curve showed that the AUC of the preoperative detection on combination of CRP,PCT and IL-6 to evaluate the treatment effect on neonatal pneumonia was 0.904,which was higher than the 0.628,0.653 and 0.777 of the three single indicators.ConclusionsThe abnormal expression of serum CRP,PCT and IL-6 levels in children with neonatal pneumonia has a certain correlation with the therapeutic effect on pneumonia,and the three can be used to assist in evaluating the therapeutic effect on neonatal pneumonia.
