倍他司汀片联合Epley耳石复位法治疗良性阵发性位置性眩晕的效果观察

摘　　要：目的分析良性阵发性位置性眩晕临床治疗中采用倍他司汀片与Epley耳石复位法联合方案治疗的临床效果。方法队列研究。抽取2021年2月至2023年2月于洛阳市中心医院接受诊疗的80例良性阵发性位置性眩晕患者为研究对象,以电脑随机数字表法分为对照组和观察组,每组40例。对照组采用Epley耳石复位法进行治疗,观察组予以倍他司汀片与Epley耳石复位法联合方案治疗。比较两组临床疗效、眩晕症状缓解效果[眩晕评定量表评分系统(DARS)、眩晕残障程度筛查量表(DHI-S)]、脑动脉血流速度及前庭症状[前庭症状指数(VSI)]改善情况。结果观察组总有效率(95.00%,38/40)高于对照组(75.00%,30/40),P<0.05。治疗后,观察组DARS与DHI-S评分均低于对照组(P均<0.05)。治疗后,观察组两侧椎动脉及基底动脉血流速度均快于对照组(P均<0.05)。治疗后,观察组VSI评分[(21.64±4.45)分]低于对照组[(31.19±6.66)分],P<0.05。结论倍他司汀片与Epley耳石复位法联合方案治疗良性阵发性位置性眩晕可取得显著疗效,不仅能提升治疗效果,促进眩晕症状缓解,而且还能加快脑动脉血流速度,提高前庭症状改善水平。ObjectiveTo analyze the clinical effect of betahistine tablet combined with Epley otolith reduction in the treatment of benign paroxysmal positional vertigo.MethodsEighty patients with benign paroxysmal positional vertigo who were treated in Luoyang Central Hospital from February 2021 to February 2023 were selected for the cohort study.And they were divided into a control group and an observation group based on computer random number table method,with 40 cases in each group.Patients in the control group were treated by Epley otolith reduction method,and patients in the observation group were treated with betahistine tablets combined with Epley otolith reduction method.The clinical efficacy,dizziness symptom relief effect assessed by dizziness and anxiety rating scale(DARS)and dizziness handicap inventory scale(DHI-S),cerebral artery blood flow velocity,and improvement level of vestibular symptom assessed by vestibular symptom index(VSI)were compared between the two groups.ResultsAfter treatment,the total effective rate in the observation group was 95.00%(38/40),higher than the 75.00%(30/40)in the control group(P<0.05).After treatment,the DARS and DHI-S scores in the observation group were lower than those in the control group(both P<0.05).After treatment,the blood flow velocities of bilateral vertebral arteries and basilar artery of the observation group were faster than those of the control group(all P<0.05).After treatment,the VSI score of the observation group(21.64±4.45)was lower than that of the control group(31.19±6.66),P<0.05.ConclusionsThe combination of betahistine tablets and Epley otolith reduction in the treatment of benign paroxysmal positional vertigo can achieve significant curative effect,which can not only improve the therapeutic effect and promote the relief of vertigo symptoms,but also accelerate the cerebral artery blood flow velocity and improve the improvement level of vestibular symptoms.
