摘 要:目的探讨心电图(ECG)复极参数诊断老年急性心肌梗死(AMI)的价值及与心功能的关系。方法回顾性抽取2021年1月至2023年12月郑州市中心医院收治的老年AMI患者92例作为AMI组,并以1∶1配比抽取同期因胸痛、气短等入院的非AMI患者92例作为非AMI组。两组均进行ECG和心功能指标[左室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)]检测,比较两组ECG复极相关参数和心功能指标,并分析其相关性。结果AMI组胸导联和肢体导联QTc间期、QT间期、T波时间长于非AMI组,AMI组LVEF水平低于非AMI组,LVESD、LVEDD水平高于非AMI组(P<0.05)。Pearson分析结果显示,老年AMI患者ECG复极参数(胸导联及肢体导联的QTc间期、QT间期、T波时间)与LVEF呈负相关(r=-0.61、-0.59、-0.53、-0.49、-0.45、-0.50,P均<0.05),与LVESD(r=0.59、0.57、0.49、0.52、0.49、0.52,P均<0.05)、LVEDD呈正相关(r=0.58、0.52、0.47、0.49、0.46、0.47,P均<0.05)。结论老年AMI患者的QTc间期、QT间期、T波时间等ECG复极参数明显延长,且与心功能存在明显关联性,临床可通过积极检测ECG复极参数来辅助诊断AMI,判断患者病情进展情况,为开展针对性治疗提供参考依据。ObjectiveTo investigate the value of electrocardiographic(ECG)repolarization parameters in the diagnosis of acute myocardial infarction(AMI)in elderly patients,and to analyze its relationship with cardiac function.MethodsA total of 92 elderly patients with AMI admitted to Zhengzhou Central Hospital from January 2021 to December 2023 were retrospectively selected as the AMI group,and 92 non-AMI patients admitted to the hospital due to chest pain and shortness of breath during the same period were selected as the non-AMI group at a ratio of 1∶1.ECG and cardiac function indexes examinations left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD)and left ventricular end diastolic diameter(LVEDD),were performed on both groups.The ECG repolarization parameters and cardiac function indexes of the two groups were compared,and their correlations were analyzed.ResultsThe QTc interval,QT interval and T wave time of thoracic leads and limb leads in the AMI group were longer than those in the non-AMI group,while the AMI group had lower LVEF,higher LVESD and higher LVEDD compared with the non-AMI group(P<0.05).The results of Pearson analysis showed that ECG repolarization parameters,such as QTc interval,QT interval,and T wave time in thoracic and limb leads,were negatively correlated with LVEF(r=-0.61,-0.59,-0.53,-0.49,-0.45,-0.50;all P<0.05),and were positively correlated with LVESD(r=0.59,0.57,0.49,0.52,0.49,0.52;all P<0.05)and LVEDD(r=0.58,0.52,0.47,0.49,0.46,0.47;all P<0.05).ConclusionsECG repolarization parameters,such as QTc interval,QT interval and T wave time,in elderly patients with AMI are significantly prolonged,and there are significant correlations with cardiac function.In clinical practice,ECG repolarization parameters can be actively detected to assist the diagnosis of AMI,judge the progression of disease,and provide reference for targeted treatment.
