人工听觉脑干植入患儿父母创伤后成长体验的质性研究

摘　　要：目的深入探究人工听觉脑干(auditory brainstem implantation,ABI)植入患儿父母创伤后成长(posttraumatic growth,PTG)的相关经历、体验。方法根据目的抽样原则,选取2023年7-9月于上海市第九人民医院听力中心进行ABI调机的14例患儿父母作为访谈对象,应用Colaizzi 7步法分析归纳主题。结果提炼出5个主题,包括疾病确诊到积极治疗的初期阶段1个主题:个人积极潜能的调动(不放弃一线希望、展现乐观思维);围术期到语训期的康复阶段3个主题:认知系统的调整(接受新生活、重建价值观),人际关系的改善(驱动同理心、社交获益),人生目标的重新设定(改变优先级、关注听力健康);贯穿于初期和康复阶段的1个主题:支持应对系统的增强(社会支持系统增强、情感支持系统增强)。结论ABI植入患儿父母在与创伤事件抗争过程中,能体验到正向的心理变化,临床医务工作者应关注家属的积极体验,把握不同阶段的关键工作,尤其要帮助父亲角色的有效应对,从而使患儿父母的PTG水平得到提升。Objective To deeply explore the relevant experience of the posttraumatic growth(PTG)of parents of children with artificial auditory brainstem implantation(ABI).Methods According to the principle of objective sampling,14 parents of the children with artificial ABI in the hearing center of Shanghai Ninth People's Hospital from July to September 2023 were selected as the interviewees,and Colaizzi 7 step was used to analyze and summarize the themes.Results There was extracted 5 themes,including one theme from disease diagnosis to the early stage of positive treatment was personal positive potential transfer(do not give up a glimmer of hope,show optimistic thinking);Three themes from perioperative period to training rehabilitation phase were cognitive system adjustment(accept new life,rebuild values),the improvement of interpersonal relationship(social benefits,drive empathy),and life goal reset(change priorities,focus on hearing health);One theme throughout the early and rehabilitation stage was support response system(social support system,emotional support system).Conclusion Parents of children with artificial ABI can experience positive psychological changes in the process of fighting with traumatic events,and clinical medical workers should pay attention to the positive experience of family members,grasp the key work at different stages,especially to effectively cope with the role of fathers,so as to promote the PTG level of parents of children.
