摘 要:目的系统整合心力衰竭患者及照顾者左心室辅助装置(left ventricular assist device,LVAD)植入决策体验的质性研究。方法检索PubMed、Embase、The Corhrane Library、Web of Science、CINAHL、万方数据、中国知网、维普数据库和中国生物医学文献服务系统中有关LVAD植入决策体验的质性研究,检索时限为建库至2023年12月1日。结果共纳入10篇文献,提取24个主题,共归纳为9个类别,合并为3个整合结果:LVAD决策思考;LVAD决策影响因素;LVAD决策需求与支持。结论患者及照顾者在进行LVAD植入决策时决策思考复杂,受多种因素影响,决策需求多样,提示医护人员应积极关注患者及照顾者决策心理,及时干预或疏导,并不断完善决策服务,提升决策服务水平,协助患者做出最佳决策,提高患者生活质量。Objective To systematically integrate the decision-making experience of left ventricular assist device(LVAD)implantation in patients with heart failure patients and their caregivers.Methods The qualitative research of LVAD implantation decision-making experience in PubMed,Embase,The Corhrane Library,Web of Science,CINAHL,Wanfang data,China National Knowledge Network,VIP database and China biomedical literature service system were searched,and the search time was from the establishment of the database to December 1,2023.Results A total of 10 pieces of literature were included,and a total of 24 topics were extracted and summarized into 9 categories,which were combined into 3 integrated results:LVAD decision thoughts,LVAD decision-making-influencing factors,LVAD decision-making needs and support.Conclusion The decision-making thinking of patients and caregivers for LVAD implantation is complex,influenced by many factors,and the decision-making needs are diverse.It is suggested that health care providers should pay attention to the decision-making psychology of patients and caregivers,timely intervention or counseling,and constantly improve decision-making services,improve the decision-making service level,assist patients to make the best decision,and improve the quality of life of patients with heart failure.
