摘　　要：燕京赵氏皮科流派对血管相关疾病的中医论治有独具特色的诊疗方法与学术观点,认为青斑样血管炎的病因病机乃先天不足、脾肾阳虚,卫外不固、风寒湿邪乘虚而入,阻于脉络,久瘀化热,蕴而成毒,内舍于脏腑,脏腑功能失调,以虚实夹杂为多,治宜扶正祛邪、标本兼治。本病病位主要在血脉肌肤,局部症状明显,阳虚寒凝血瘀证最为常见,故诊治上重视治标与治本兼施,整体与局部兼顾,辨病与辨证相结合的诊疗思想,确立了扶正祛邪为主的治则,治以温阳活血通络法,采取中药内服以活血散瘀汤、凉血五根汤为基础方加减扶正祛邪,减少复发,同时配合以中药泡洗以求急袪其邪,减轻局部症状。中药内外合用,标本兼顾治疗该病取得了良好临床疗效。The Yanjing Zhao School of Dermatology has unique diagnostic and therapeutic methods as well as academic views on vascular-related diseases in traditional Chinese medicine.It believes that the etiology and pathogenesis of this disease are primarily due to congenital deficiency,deficiency of spleen and kidney yang,weakness of defensive qi,invasion of wind,cold,and dampness evils due to the deficiency,obstruction in the meridians and collaterals,long-term stasis transforming into heat,and accumulation into toxins,which lodge in the viscera causing dysfunction.The syndrome differentiation mostly involves a mixture of deficiency and excess,thus treatment emphasizes supporting the healthy qi while dispelling evils,addressing both the root and the branch of the illness.The main location of the disease is in the blood vessels,muscles,and skin,with prominent local symptoms.The syndrome of yang deficiency,cold coagulation,and blood stasis is most common,so treatment focuses on addressing both the symptoms and the root cause,combining overall and local considerations,and integrating disease differentiation with syndrome differentiation.This establishes the principle of supporting the healthy Qi while dispelling evils,with the main emphasis on dispelling evils.The treatment method primarily involves warming yang,promoting blood circulation,and dredging collaterals.It employs Chinese herbal medicine internally,based on formulas such as Huoxue Sanyu Decoction(Blood-Moving and StasisDissipating Decoction)and Liangxue Wugen Decoction(Blood-Cooling Five Roots Decoction),with modifications to support the healthy Qi while dispelling evils,reducing recurrence.Simultaneously,external application of Chinese herbal medicine is used to rapidly eliminate evils and alleviate local symptoms.The combination of internal and external use of Chinese herbal medicine,with consideration of both the root and the branch of the illness,has achieved good clinical efficacy in the treatment of this disease.
