益肾疏肝起痿汤联合他达拉非治疗勃起功能障碍肝郁肾虚型的疗效及对血清性激素和血管内皮功能的影响

摘　　要：目的探讨益肾疏肝起痿汤联合他达拉非治疗勃起功能障碍(erectile dysfunction,ED)肝郁肾虚型的疗效及对血清性激素和血管内皮功能的影响。方法ED患者80例随机分为对照组与观察组,每组40例。对照组予以他达拉非治疗,观察组予以益肾疏肝起痿汤联合他达拉非治疗,两组均连续治疗4周。治疗前和治疗后对患者进行勃起功能国际问卷-5(international index of erectile function-5,IIEF-5)评分、勃起质量量表(erectile quality scale,EQS)评分、勃起硬度(erectile hardness,EHS)评分及中医证候积分,进行血清性激素[睾酮(testosterone,T)、促黄体生成素(luteinizing hormone,LH)、卵泡刺激素(follicle stimulating hormone,FSH)]和血管内皮功能指标[内皮素-1(endothelin-1,ET-1)、一氧化氮(nitric oxide,NO)]的测定,比较两组临床疗效和各项指标的差异,并观察不良反应。结果观察组治疗总有效率为90.00%(36/40),与对照组(72.50%,29/40)比较显著增高(P<0.05)。治疗后,观察组IIEF-5评分、EQS评分、EHS评分更高(P<0.05),中医证候积分更低(P<0.05)。治疗后,观察组血清T水平高于对照组(P<0.05),LH和FSH水平低于对照组(P<0.05)。治疗后,观察组血清NO水平高于对照组(P<0.05),ET-1水平低于对照组(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论益肾疏肝起痿汤联合他达拉非治疗ED肝郁肾虚型患者疗效显著,能够改善患者勃起功能,其机制可能与调节性激素和血管内皮功能有关。Objective To investigate the efficacy of Yishen Shugan Qiwei Decoction(益肾疏肝起痿汤)combined with tadalafil in the treatment of erectile dysfunction(ED)with liver depression and kidney deficiency and its effect on serum sex hormones and vascular endothelial function.Methods A total of 80 ED patients were randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with tadalafil,and the observation group was treated with Yishen Shugan Qiwei Decoction combined with tadalafil.Both groups were treated continuously for 4 weeks.Before and after treatment,the International Index of Erectile Function-5(IIEF-5)score,Erectile Quality Scale(EQS)score,Erectile Hardness(EHS)score and TCM syndrome score were performed on the patients.Serum sex hormones[testosterone(T),luteinizing hormone(LH),follicle stimulating hormone(FSH)]and vascular endothelial function indicators[endothelin-1(ET-1),nitric oxide(NO)]were measured.The clinical efficacy and differences in various indicators were compared between the two groups,and adverse reactions were observed.Results The total effective rate of the observation group was 90.00%(36/40),which was significantly higher than that of the control group(72.50%,29/40)(P<0.05).After treatment,the IIEF-5 score,EQS score and EHS score of the observation group were higher(P<0.05),and the TCM syndrome score was lower(P<0.05).After treatment,the serum T level in the observation group was higher than that in the control group(P<0.05),and the levels of LH and FSH were lower than those in the control group(P<0.05).After treatment,the serum NO level in the observation group was higher than that in the control group(P<0.05),and the ET-1 level was lower than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Yishen Shugan Qiwei Decoction combined with tadalafil is effective in the treatment of ED patients with liver depression and kidney deficiency
