痰热清注射液联合清燥救肺汤加减对放射性肺损伤的预防效果分析

摘　　要：目的探讨痰热清注射液联合清燥救肺汤加减对放射性肺损伤的预防效果。方法选取100例接受胸部三维适形放射治疗的肺癌患者,随机分两组,各50例。对照组单纯放射治疗,观察组在此基础上应用痰热清联合清燥救肺汤加减。比较两组治疗前后临床疗效、放射性肺损伤发生情况及持续时间、血细胞分析、肺功能及生活质量、放疗后1、2、3、6、12个月的中医证候积分、胸部CT影像学表现。结果观察组总有效率显著高于对照组(P<0.05);观察组肺损伤发生率和持续时间均显著小于对照组(P<0.05);炎症因子水平均显著改善,观察组优于对照组(P<0.05);肺功能指标均显著提升,观察组优于对照组(P<0.05);观察组生活质量卡氏评分治疗后显著提升,优于对照组(P<0.05);CT值及证候评分均显著改善,观察组优于对照组(P<0.05)。结论痰热清注射液联合清燥救肺汤加减治疗放射性肺损伤具有良好预防效果,降低放射性肺损伤程度及持续时间,改善患者临床症状及CT影像,降低炎症因子水平,提升生活质量。Objective To explore the preventive effect of Tanreqing Injection(痰热清注射液)combined with modified Qingzao Jiufei Decoction(清燥救肺汤)on radiation-induced lung injury.Methods A total of 100 patients with lung cancer who received chest three-dimensional conformal radiotherapy were selected and randomly divided into two groups,50 cases in each group.The control group received radiotherapy alone,while the observation group received Tanreqing Injection combined with modified Qingzao Jiufei Decoction on this basis.The clinical curative effect before and after treatment,the incidence and duration of radiation lung injury,blood cell analysis,lung function and quality of life,traditional Chinese medicine syndrome scores 1,2,3,6 and 12 months after radiotherapy,and chest CT imaging findings were compared between the two groups.Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The incidence and duration of lung injury in the observation group were significantly less than those of the control group(P<0.05).The levels of inflammatory factors were significantly improved,and the improvement of the observation group was better than that of the control group(P<0.05).The pulmonary function indicators were significantly improved,and the indicators in the observation group were better than those of the control group(P<0.05).The Karnofsky score of the quality of life in the observation group was significantly improved after treatment,which was better than that of the control group(P<0.05).Value and symptom scores were significantly improved,and the improvement of the observation group was better than that of the control group(P<0.05).Conclusion Tanreqing Injection combined with modified Qingzao Jiufei Decoction has a good preventive effect on radiation-induced lung injury,reduces the degree and duration of radiation-induced lung injury,improves clinical symptoms and CT images,reduces the levels of inflammatory factors,and improves the quality of l
