虎杖汤灌肠联合大黄芒硝散脐周外敷对轻中度急性脂源性胰腺炎患者脂联素及肠功能恢复的影响

摘　　要：目的探讨虎杖汤灌肠联合大黄芒硝散脐周外敷对轻中度急性脂源性胰腺炎患者脂联素及肠功能恢复的影响。方法选择2020年1月—2022年12月收治的轻中度急性脂源性胰腺炎患者共计90例,根据随机对照分组,分为试验组(45例)、对照组(45例)。对照组予以常规干预措施,试验组予以虎杖汤灌肠联合大黄芒硝散脐周外敷。比较两组临床疗效、肠功能恢复时间、中医证候积分、血清C反应蛋白(C-reactive protein,CRP)、脂联素、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平、不良反应发生情况。应用SPSS 24.0软件进行统计学分析。结果对照组有效率73.33%(33/45)低于试验组95.56%(43/45)(P<0.05)。试验组自主排便的时间、腹痛与腹胀症状消失的时间缩短(P<0.05)。两组治疗后腹胀腹痛、大便黏滞不通、胸闷不舒、烦渴引饮、小便短黄、身目发黄等中医证候积分降低(P<0.05);与对照组比较,试验组治疗后各项中医证候积分较低(P<0.05)。与治疗前比较,两组治疗后血清CRP、TNF-α水平降低,脂联素水平升高;与对照组比较,试验组治疗后血清CRP、TNF-α水平较低,脂联素水平较高(P<0.05)。两组不良反应比较差异无统计学意义。结论虎杖汤灌肠联合大黄芒硝散脐周外敷能够显著缩短轻中度急性脂源性胰腺炎肠功能恢复,提高临床疗效,抑制炎症反应,适宜临床应用推广。Objective To investigate the effects of Huzhang Decoction(虎杖汤)enema combined with Dahuang Mangxiao Powder(大黄芒硝散)on the recovery of adiponectin and intestinal function in patients with mild to moderate acute lipogenic pancreatitis.Methods A total of 90 patients with mild to moderate acute lipogenic pancreatitis treated from January 2020 to December 2022 were selected and divided into experimental group(45 cases)and control group(45 cases)according to randomized control group.The control group was given routine intervention measures,and the experimental group was given Huzhang Decoction enema combined with Dahuang Mangxiao Powder around the umbilicus.The clinical efficacy,intestinal function recovery time,TCM syndrome score,serum C-reactive protein(CPR),adiponectin,tumor necrosis factor-α(TNF-α)and the occurrence of adverse reactions were compared between the two groups.SPSS 24.0 software was used for statistical analysis.Results The effective rate of the control group was 73.33%(33/45),lower than that of the experimental group[95.56%(43/45)](P<0.05).Compared with those of the control group,the time of spontaneous defecation and the time of disappearance of abdominal pain and abdominal distension in experimental group were shortened(P<0.05).After treatment,the scores of TCM syndrome such as abdominal distension,abdominal pain,sticky stool,chest tightness,dyspepsia,short and yellow urine,yellow body and eye were decreased in both groups(P<0.05).Compared with those of the control group,the scores of TCM syndromes in the experimental group were lower after treatment(P<0.05).Compared with those before treatment,serum levels of CRP and TNF-αwere decreased and adiponectin levels were increased in both groups after treatment.Compared with those of the control group,the serum levels of CRP and TNF-αwere lower in the experimental group after treatment,and the levels of adiponectin were higher(P<0.05).There was no difference in adverse reactions between the two groups.Conclusion The combination of Huzhang
