蠲痛舒筋胶囊联合雷火灸治疗神经根型颈椎病疗效观察

摘　　要：目的研究蠲痛舒筋胶囊联合雷火灸治疗神经根型颈椎病的临床疗效。方法选择2021年1月—2023年3月于衡水市中医医院就诊并诊断为神经根型颈椎病患者92例,随机分为联合组(46例)、常规组(46例),所有患者均予以雷火灸联合解痉、止痛、营养神经相关药物干预,配合牵引、舒筋理疗干预,联合组另加用蠲痛舒筋胶囊干预,疗程14 d。比较干预前后的Northwick Park颈痛评分(Northwick Park Questionnaire,NPQ)、中医症状积分、颈椎功能障碍指数(neck disability index,NDI)、电生理情况、血清疼痛因子水平、不良反应情况及临床疗效。结果干预14 d后,患者NPQ评分、中医症状积分、NDI评分、血清疼痛因子水平均较干预前降低,联合组降幅大于常规组(P<0.05);干预14 d后,患者腕、肘尺神经和正中神经F波传递速度联合组优于常规组(P<0.05);干预14 d后,联合组临床疗效(46/46,100.00%)优于常规组(39/46,84.78%)(P<0.05);干预14 d后,联合组(6/46,13.04%)与常规组(7/46,15.21%)不良反应发生情况差异无统计学意义(P>0.05)。结论蠲痛舒筋胶囊联合雷火灸治疗神经根型颈椎病可降低患者的血清疼痛因子水平,治疗后患者的颈椎活动度更佳,还可以改善患者的临床证候,提升疗效,值得推广。Objective To study the clinical effect of Juantong Shujin Capsule(蠲痛舒筋胶囊)combined with thunder-fire moxibustion in the treatment of radiculopathy of cervical spondylosis.Methods A total of 92 patients diagnosed with radiculopathy in the hospital from January 2021 to March 2023 were selected and randomly divided into the combination group(46 cases)and the conventional group(46 cases).All patients were treated with spasmolysis,analgesic and neurotrophic drugs,combined with traction and tendon-soothing physiotherapy.Juantong Shujin Capsule combined with thunder fire moxibustion intervention was also used in the combination group for 14 days.The Northwick Park Questionnaire(NPQ),TCM symptom score,neck disability index(NDI),electrophysiological status,serum pain factors,adverse reactions and clinical efficacy were compared before and after the intervention.Results After 14 days of intervention,NPQ score,TCM symptom score,NDI score and serum pain factor levels were all decreased compared with before intervention,and the decrease in the combination group was greater than that in the conventional group(P<0.05).After 14 days of intervention,the F-wave transmission velocity of wrist,ulnar nerve and median nerve in the combination group was better than that in the conventional group(P<0.05).After 14 days of intervention,the clinical effect of the combination group(46/46,100.00%)was better than that of the conventional group(39/46,84.78%)(P<0.05).After 14 days of intervention,there was no difference in the occurrence of adverse reactions between the combination group(6/46,13.04%)and the conventional group(7/46,15.21%)(P>0.05).Conclusion Juantong Shujin Capsule combined with thund-fire moxibustion in the treatment of cervical radiculopathy can reduce the levels of serum pain factors of patients and improve the cervical motion,the clinical symptoms and the curative effect,which is worthy of promotion.
