摘 要:目的:探讨经鼻高流量氧疗(HFNC)在支气管镜诊疗患者深度镇静中的应用效果。方法:选取2021年1月—2024年3月于扬中市人民医院呼吸内科行支气管镜诊疗的患者100例作为研究对象,按照随机数字表法分为试验组和对照组,各50例。两组均给予深度镇静,试验组实施HFNC,对照组实施鼻导管吸氧。比较两组生命体征指标、改良警觉/镇静评分(MOAA/S)、气道反应评分、主观感受评分、呼吸功能指标、不良反应发生情况。结果:麻醉中,试验组平均动脉压、心率、呼吸频率均低于对照组(P<0.001)。两组MOAA/S评分、气道反应评分、主观感受评分比较,差异无统计学意义(P>0.05)。麻醉中,试验组经皮血氧饱和度、氧分压高于对照组,二氧化碳分压低于对照组(P<0.001)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:HFNC在支气管镜诊疗患者深度镇静中的应用效果显著,能够稳定患者生命体征,改善氧合。Objective:To explore the application effect of high-flow nasal cannula oxygen therapy(HFNC)in deep sedation of patients undergoing bronchoscopic diagnosis and treatment.Methods:A total of 100 patients who underwent bronchoscopic diagnosis and treatment in the Respiratory Department at the People's Hospital of Yangzhong City from January 2021 to March 2024 were selected as the study subjects.They were divided into an experimental group and a control group using a random number table method,with 50 cases in each group.Both groups received deep sedation.The experimental group received HFNC,while the control group received nasal cannula oxygen inhalation.Vital signs,Modified Observer's Assessment of Alertness and Sedation(MOAA/S)scale score,airway response score,subjective perception score,respiratory function index,and occurrence of adverse reactions were compared between the two groups.Results:During anesthesia,the mean arterial pressure,heart rate and respiratory rate in the experimental group were lower than those in the control group(P<0.001).There was no significant difference in MOAA/S score,airway response score and subjective perception score between the two groups(P>0.05).During anesthesia,the transcutaneous oxygen saturation and oxygen partial pressure in the experimental group were higher than those in thecontrolgroup,and the partial pressure of carbon dioxide in the experimental group was lower than that in the control group(P<0.001).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:HFNC has a significant effect in deep sedation of patients undergoing bronchoscopic diagnosis and treatment,and can stabilize patients'vital signs and improve oxygenation.
