摘　　要：目的:探讨长期深静脉留置导管在慢性肾功能衰竭患者维持性血液透析中的应用效果。方法:选取2020年1月—2023年1月于南京医科大学附属无锡人民医院血液净化中心接受长期血液透析的100名慢性肾功能衰竭患者作为研究对象,随机分为对照组和观察组,各50例。两组均采用动静脉内瘘血液透析治疗,对照组未留置深静脉导管,观察组长期留置深静脉导管。比较两组微炎症指标、营养状态评分、实验室检查指标及并发症发生情况。结果:治疗后,观察组微炎症指标、营养状态评分、实验室指标均优高于对照组(P<0.05)。观察组并发症总发生率低于对照组(P=0.025)。结论:长期深静脉留置导管在慢性肾功能衰竭患者维持性血液透析中的应用效果较好,能够改善患者的营养状态,提高血清白蛋白、总蛋白水平,降低肌酐、总胆固醇水平和并发症总发生率,但会提高微炎症指标水平,临床应根据患者的具体情况留置深静脉导管。Objective:To investigate the effect of long-term deep vein catheterization in maintenance hemodialysis of patients with chronic renal failure.Methods:A total of 100 patients with chronic renal failure who underwent long-term hemodialysis at the Blood Purification Center of Wuxi People's Hospital Affiliated to Nanjing Medical University from January 2020 to January 2023 were selected as the study subjects.They were randomly divided into control group and observation group,with 50 cases in each group.Both groups received hemodialysis treatment with internal arteriovenous fistula.In the control group,no deep vein catheterization was performed.In the observation group,long-term deep vein catheterization was performed.The microinflammation indexes,nutritional status scores,laboratory examination indexes and complications were compared between the two groups.Results:After treatment,microinflammatory indicators,nutritional status scores,and laboratory indicators were better in the observation group than in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P=0.025).Conclusion:Long-term deep vein catheterization has a good effect on maintenance hemodialysis of patients with chronic renal failure,and can improve the nutritional status of patients,increase the levels of serum albumin and total protein,reduce the levels of creatinine and total cholesterol and the total incidence of complications,but may increase the levels of microinflammation indicators.In clinical setting,deep vein catheterization is performed according to specific condition of patients.
