关节镜下关节清理扩创微骨折术联合术后康复训练在中老年膝骨关节炎患者中的应用效果分析

摘　　要：目的:分析关节镜下关节清理扩创微骨折术联合术后康复训练在中老年膝骨关节炎患者中的应用效果。方法:选取2020年1月—2021年7月台山市人民医院收治的膝骨关节炎患者94例作为研究对象,采用随机数字表法分为对照组和观察组,每组47例。对照组实施关节镜下关节清理扩创微骨折术,观察组在对照组基础上实施康复训练。比较两组膝关节功能、炎性因子水平、生活质量、临床疗效。结果:术后1、3、6个月,两组膝关节功能评分高于术前,且观察组高于对照组(P<0.05)。术后1个月,两组脂联素、转化生长因子-β1、白细胞介素-17水平低于术前,且观察组低于对照组(P<0.05);两组总体健康、生理功能、精神健康、活力、社会功能评分高于术前,且观察组高于对照组(P<0.05)。观察组治疗总有效率高于对照组(P=0.003)。结论:关节镜下关节清理扩创微骨折术联合术后康复训练在中老年膝骨关节炎患者中的应用效果显著,可改善患者膝关节功能,减轻炎性反应,提高患者生活质量。Objective:To analyze the application effect of arthroscopic joint debridement and microfracture combined with postoperative rehabilitation training in middle-aged and elderly patients with knee osteoarthritis.Methods:A total of 94 patients with knee osteoarthritis who visited Taishan People's Hospital from January 2020 to July 2021 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 47 cases in each group.The control group received arthroscopic joint debridement and microfracture and the observation group received rehabilitation training on the basis of control group.Knee joint function,levels of inflammatory factors,quality of life and clinical efficacy were compared between the two groups.Results:At 1,3 and 6 months after surgery,the knee joint function scores in the two groups were higher than those before surgery,and the scores in the observation group were higher than those in the control group(P<0.05).At 1 month after surgery,the levels of adiponectin,transforming growth factor-β1 and interleukin-17 in the two groups were lower than those before surgery,and the levels in the observation group were lower than those in the controlgroup(P<0.05).The scores of general health,physiological function,mental health,vitality and social function in the two groups were higher than those before surgery,and the scores in the observation group were higher than those in the control group(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group(P=0.003).Conclusion:Arthroscopic joint debridement and microfracture combined with postoperative rehabilitation training in middle-aged and elderly patients with knee osteoarthritis has a significant effect,and can improve knee joint function,relieve inflammatory reaction and improve the quality of life of patients.
