动脉血气分析联合床旁肺部超声在重症肺炎合并呼吸衰竭诊断中的应用价值

摘　　要：目的:动脉血气分析联合床旁肺部超声在重症肺炎合并呼吸衰竭诊断中的应用价值。方法:选取2023年1月—2024年8月佛山市第五人民医院收治的80例疑似重症肺炎合并呼吸衰竭患者作为研究对象,患者均进行胸部CT检查、动脉血气分析及床旁肺部超声检查,以胸部CT检查结果为诊断“金标准”,比较动脉血气分析、床旁肺部超声及动脉血气分析联合床旁肺部超声检查诊断重症肺炎合并呼吸衰竭的诊断效能。结果:胸部CT检查结果显示,80例疑似重症肺炎合并呼吸衰竭患者中确诊为重症肺炎合并呼吸衰竭72例,动脉血气分析诊断为重症肺炎合并呼吸衰竭59例,床旁肺部超声检查诊断为重症肺炎合并呼吸衰竭63例,动脉血气分析联合床旁肺部超声检查诊断为重症肺炎合并呼吸衰竭75例。动脉血气分析联合床旁肺部超声检查诊断的灵敏度、准确度高于动脉血气分析、床旁肺部超声检查(P<0.05)。结论:动脉血气分析联合床旁肺部超声检查诊断重症肺炎合并呼吸衰竭的临床价值较高,值得临床推广并予以应用。Objective:To explore the application value of arterial blood gas analysis combined with bedside pulmonary ultrasound in the diagnosis of severe pneumonia complicated with respiratory failure.Methods:A total of 80 patients with suspected severe pneumonia complicated with respiratory failure admitted to Foshan No.5 People's Hospital from January 2023 to August 2024 were selected as the study subjects.All patients underwent chest CT,arterial blood gas analysis and bedside pulmonary ultrasound examination.Chest CT examination results were used as the diagnostic"gold standard"to compare the diagnostic efficiency of arterial blood gas analysis,bedside pulmonary ultrasound,and arterial blood gas analysis combined with bedside pulmonary ultrasound for the diagnosis of severe pneumonia complicated with respiratory failure.Results:Among 80 patients with suspected severe pneumonia complicated with respiratory failure,72 cases had severe pneumonia complicated with respiratory failure by Chest CT examination,59 cases had severe pneumonia complicated with respiratory failure by arterial blood gas analysis,63 cases had severe pneumonia complicated with respiratory failure by bedside pulmonary ultrasound examination,and 75 cases had severe pneumonia complicated with respiratory failure by arterial blood gas analysis combined with bedside pulmonary ultrasound.The diagnostic sensitivity and accuracy of arterial blood gas analysis combined with bedside pulmonary ultrasound were higher than those of arterial blood gas analysis and bedside pulmonary ultrasound(P<0.05).Conclusion:Arterial blood gas analysis combined with bedside pulmonary ultrasound has high clinical value in the diagnosis of severe pneumonia complicated with respiratory failure,and deserves clinical promotion and application.
