手法复位夹板外固定术联合外敷四黄散治疗肱骨髁上骨折患者的临床效果

摘　　要：目的探讨手法复位夹板外固定术联合外敷四黄散治疗肱骨髁上骨折患者的临床效果。方法选取2020年3月至2024年4月广州医科大学附属第四医院收治的80例肱骨髁上骨折患者作为研究对象,按照随机数字表法分为对照组(40例)和观察组(40例)。对照组实施手法复位+石膏外固定治疗,观察组采用手法复位+夹板外固定+外敷四黄散治疗,比较两组的临床治疗优良率、肘关节功能、肘关节活动度、治疗时间、骨折愈合时间和并发症发生情况。结果观察组的临床治疗优良率(95.00%)高于对照组(80.00%),差异有统计学意义(P<0.05);两组患者治疗后的各项肘关节Mayo功能评分(MEPS)均高于本组治疗前,且观察组治疗后的各项MEPS均高于对照组,差异有统计学意义(P<0.05);观察组的肘关节旋前、旋后及屈伸度均大于对照组,治疗时间和骨折愈合时间均短于对照组,并发症总发生率(5.00%)低于对照组(20.00%),差异有统计学意义(P<0.05)。结论手法复位夹板外固定术加外敷四黄散治疗肱骨髁上骨折效果理想,利于改善肘关节功能,促进骨折愈合,降低并发症发生率,加快康复进程,值得临床推广应用。Objective To explore the clinical effect of manual reduction and splint external fixation combined with external application of Sihuang Powder in the treatment of supracondylar fracture of humerus.Methods A total of 80 patients with supracondylar fracture of humerus admitted to the Fourth Affiliated Hospital of Guangzhou Medical University from March 2020 to April 2024 were selected as the research objects.According to the random number table method,they were divided into the control group(40 cases)and the observation group(40 cases).The control group was treated with manual reduction+plaster external fixation,and the observation group was treated with manual reduction+splint external fixation+external application of Sihuang Powder.The excellent and good rate of clinical treatment,elbow joint function,elbow joint activity,treatment time,fracture healing time and complications were compared between the two groups.Results The excellent and good rate of clinical treatment in the observation group(95.00%)was higher than that in the control group(80.00%),the difference was statistically significant(P<0.05).The Mayo elbow performance score(MEPS)of the two groups after treatment were higher than those before treatment,and the MEPS of the observation group after treatment were higher than those of the control group,and the differences were statistically significant(P<0.05).The elbow pronation,supination,and flexion and extension of the observation group were bigger than those of the control group,the treatment time and fracture healing time were shorter than those of the control group,and the total incidence of complications(5.00%)was lower than that of the control group(20.00%),the differences were statistically significant(P<0.05).Conclusion The effect of manual reduction and splinting external fixation combined with external application of Sihuang Powder in the treatment of supracondylar fracture of humerus is ideal,which can improve the elbow joint function,promote fracture healing,reduce the incidence of complicatio
