摘  要:目的分析感染性肺炎新生儿血清淀粉样蛋白A(SAA)、25羟基维生素D_(3)[25(OH)D_(3)]、内毒素(LSP)水平与肺部超声(LUS)评分的相关性。方法回顾性分析2020年1月至2023年3月赣州市妇幼保健院收治的82例感染性肺炎新生儿(观察组)和50名进行健康体检的新生儿(对照组)的临床资料。比较两组研究对象的一般资料、LUS评分、SAA、25(OH)D_(3)、LSP水平,采取Pearson相关分析法,分析SAA、25(OH)D_(3)、LSP指标与LUS评分的相关性,绘制受试者工作特征(ROC)曲线,计算敏感度、特异度、曲线下面积(AUC)、约登指数并确定最佳截断值,比较各指标单独及联合对新生儿感染性肺炎的诊断价值。结果观察组的LUS评分、SAA、LSP水平均高于对照组,25(OH)D_(3)水平低于对照组,差异有统计学意义(P<0.05);相关性分析显示,LUS评分与SAA、LSP之间均为正相关关系(r=0.565、0.464,P<0.05),与25(OH)D_(3)之间为负相关关系(r=-0.399,P<0.05);联合LUS评分、SAA、LSP、25(OH)D_(3)指标对新生儿感染性肺炎的诊断敏感度、特异度、约登指数、AUC为0.988、0.987、0980、0.998,明显高于LUS评分、SAA、LSP、25(OH)D_(3)指标单独诊断。结论感染性肺炎新生儿的LUS评分与SAA、25(OH)D_(3)、LSP水平呈相关性,各指标对新生儿感染性肺炎均具有一定的诊断价值,且LUS评分联合SAA、25(OH)D_(3)、LSP的诊断价值高于各个指标的单独诊断价值。通过上述指标联合能够尽早对患儿进行诊断,及时采取有效措施进行干预,为提高临床疗效提供理论指导。Objective To analyze the correlation between serum amyloid A(SAA),25-hydroxyvitamin D_(3)(25[OH]D_(3)),lipopolysaccharides(LSP)levels and lung ultrasound(LUS)score in neonates with infectious pneumonia.Methods The clinical data of 82 neonates with infectious pneumonia(observation group)and 50 neonates who underwent physical examination(control group)in Ganzhou Maternal and Child Health Hospital from January 2020 to March 2023 were retrospectively analyzed.The general data,LUS score,SAA,25(OH)D_(3),and LSP levels of the two groups were compared.Pearson correlation analysis was used to analyze the correlation between SAA,25(OH)D_(3),LSP indexes and LUS score,and the receiver operating characteristic(ROC)curve was drawn.The sensitivity,specificity,area under the curve(AUC),and Youden index were calculated,and the best cut-off value was determined.The diagnostic value of each index alone and in combination for neonatal infectious pneumonia were compared.Results The LUS score,SAA and LSP levels of the observation group were higher than those of the control group,and the 25(OH)D_(3) level was lower than that of the control group,and the differences were statistically significant(P<0.05).Correlation analysis showed that LUS score was positively correlated with SAA and LSP(r=0.565,0.464,P<0.05),and negatively correlated with 25(OH)D_(3)(r=-0.399,P<0.05).The sensitivity,specificity,Youden index and AUC value of the combination of LUS score,SAA,LSP and 25(OH)D_(3) in the diagnosis of neonatal infectious pneumonia were 0.988,0.987,0980 and 0.998,respectively,which were significantly higher than those of LUS score,SAA,LSP and 25(OH)D_(3) alone.Conclusion In neonates with infectious pneumonia,LUS score is correlated with SAA,
