基于补肾活血法探讨国医大师张大宁教授运用角药治疗慢性肾脏病的经验

摘　　要：慢性肾脏病是临床常见多发病,如果不及时有效治疗,其病情不断进展,最终可导致尿毒症,即终末期肾病。中医药在治疗慢性肾脏病方面有独特的优势,其治疗原则和方法与西医有所不同,注重整体调理和辨证施治。国医大师张大宁教授提出的“肾虚血瘀论”与“补肾活血法”是在长期临床实践基础上,针对慢性肾脏病的病理特点所形成的一种治疗策略。该理论认为,不同的致病因子所导致的不同疾病,发展到某一阶段,都会出现相同的病理改变,即“肾虚血瘀”;补肾活血法通过补肾促进活血,应用活血加强补肾,两者相互协同,达到改善肾虚血瘀病理变化。在该理论的指导下,临床治疗各类慢性肾脏疾病取得了显著疗效。同时,在临床用药中,三联药配伍法(角药)是张大宁教授的特色用药方法,做到升降并用,寒热并用,脏腑同治,攻补兼施,补中寓攻,攻中无损,相济为用,临床疗效显著。Chronic kidney disease is a common and frequent disease in clinic.Without timely and effective treatment,its condition will continue to progress and eventually lead to uremia,that is,end-stage renal disease.Traditional Chinese medicine has unique advantages in the treatment of chronic kidney disease.Its treatment principles and methods are different from those of western medicine,focusing on holistic management and evidence-based treatment.Professor ZHANG Daning,a master of traditional Chinese medicine,put forward the"theory of kidney deficiency and blood stasis"and the"method of toning kidney and activating blood circulation",which is a treatment strategy based on the long-term clinical practice and the pathological characteristics of chronic kidney disease.According to this theory,different diseases caused by different pathogenic factors will have the same pathological changes when they reach a certain stage of development,that is,"kidney deficiency and blood stasis".The method of tonifying kidney and activating blood circulation can improve the pathological changes of kidney deficiency and blood stasis by tonifying kidney and strengthening tonifying kidney by activating blood circulation.Under the guidance of this theory,clinical treatment of various types of chronic kidney diseases has achieved significant effect.At the same time,In the clinical medication,the triple combination method(triplet herbal medicine)is Professor ZHANG Daning's characteristic method of medication,to achieve the elevation and use of cold and heat,the same treatment of internal organs,attack and tonic at the same time,the tonic in the attack,the attack in the attack,no loss,and the use of the mutual aid,the clinical efficacy of the treatment is remarkable.
