甲状腺球蛋白抗体、甲状腺过氧化物酶抗体检测在妊娠期甲状腺疾病诊断中的价值

摘  要:目的探究妊娠期甲状腺疾病应用甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)检测的诊断价值。方法选择2021年1月至2023年12月江门市妇幼保健院132例就诊的妊娠期甲状腺疾病患者入研究组,另选择同期行正常孕期检查的50例志愿者入对照组。抽取所有参检者静脉血,测定血清TPOAb、TGAb水平。比较两组血清TPOAb、TGAb水平、比较不同类型甲状腺疾病患者血清TPOAb、TGAb水平,另比较单独及联合检测在甲状腺疾病中的诊断价值。结果研究组血清TPOAb、TGAb水平均高于对照组,差异有统计学意义(P<0.05)。甲亢患者血清TPOAb、TGAb水平均高于甲减及甲状腺肿,且甲减患者血清TPOAb、TGAb水平高于甲状腺肿,差异有统计学意义(P<0.05)。血清TPOAb、TGAb联合检测在妊娠期甲状腺疾病中的诊断价值最高,AUC为0.933,灵敏度为0.914、特异度为0.750;其次为TGAb的AUC为0.892,灵敏度为0.840、特异度为0.767;TPOAb的AUC为0.804,灵敏度为0.801、特异度为0.650。结论在妊娠期甲状腺疾病诊断中应用TPOAb、TGAb联合检测可获得较高的临床应用价值,有助于提高妊娠期甲状腺疾病检出率,且观察各指标水平的变化有助于明确疾病类型,为临床进一步诊疗提供有效参考,具有较高的临床推广应用价值。Objective To explore the diagnostic value of thyroid globulin antibody(TGAb)and thyroid peroxidase antibody(TPOAb)detection for gestational thyroid diseases.Methods A total of 132 patients with gestational thyroid disease who received medical treatment from Jiangmen Women and Children's Health Hospital from January 2021 to December 2023 were enrolled in the study group,and 50 volunteers who underwent normal pregnancy examination during the same period were enrolled in the control group.The serum TPOAb and TGAb levels were measured by venous blood samples from all participants.The serum TPOAb and TGAb levels of the two groups were compared,and the serum TPOAb and TGAb levels of patients with different types of thyroid diseases were compared,and the diagnostic value of single and combined detection in thyroid diseases was compared.Results The serum TPOAb and TGAb levels in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Serum TPOAb and TGAb levels in hyperthyroidism patients were higher than those in hypothyroidism and goiter,and serum TPOAb and TGAb levels in hypothyroidism patients were higher than those in goiter,the differences were statistically significant(P<0.05).Serum TPOAb and TGAb combined detection had the highest diagnostic value in thyroid diseases during pregnancy,with AUC of 0.933,sensitivity of 0.914 and specificity of 0.750.The AUC of TGAb was 0.892,the sensitivity was 0.840,and the specificity was 0.767.The AUC of TPOAb was 0.804,the sensitivity was 0.801 and the specificity was 0.650.Conclusion The application of TPOAb and TGAb combined detection in the diagnosis of thyroid diseases in pregnancy can obtain high clinical application value,which helps to improve the detection rate of thyroid diseases in pregnancy,and the observation of the changes of each index level helps to identify the types of diseases,provide an effective reference for further clinical diagnosis and treatment,and has high clinical application value.
