摘 要:肺不张是小儿呼吸系统的常见并发症,其病因复杂、机制多样、治疗多元。机械通气在肺不张中具有两面性,既可以是肺不张的原因,也可以是治疗方法,掌握恰当的机械通气策略,趋利避害,是儿童肺不张防治的关键原则。本研究旨在阐述呼气末正压通气、持续气道正压通气、单肺通气等机械通气模式在儿童肺不张预防和辅助治疗中的应用进展,为临床通气方案提供参考。Pulmonary atelectasis is a common complication of the pediatric respiratory system with complex etiology,diverse mechanisms,and multifaceted treatment.Mechanical ventilation has two sides in pulmonary atelectasis,i.e.it can be both the cause of pulmonary atelectasis and a treatment method,and mastering an appropriate mechanical ventilation strategy to avoid harm is the key principle in the prevention and treatment of pulmonary atelectasis in children.The aim of the article was to briefly describe the progress of the application of mechanical ventilation modes such as positive end expiratory pressure ventilation,continuous positive airway pressure ventilation,and single-lung ventilation in the prevention and adjunctive treatment of pulmonary atelectasis in children,and to provide a basic reference for clinical ventilation programs.
