摘 要:目的探讨T组合复苏器联合注射泵肺内注入肺表面活性剂(PS)的方法在治疗患有新生儿呼吸窘迫综合征(NRDS)的极早产儿中的临床疗效。方法选取2018年1月~2020年9月川北医学院附属医院新生儿重症监护室收治的诊断明确的患有NRDS的极早产儿患者作为研究对象,研究对象均采用气管插管—气管内注入PS—拔管给予CPAP治疗,根据正压通气及推注PS的方式不同分为复苏囊加压联合注射器推药组(即传统组)共计60例和T组合复苏器加压联合注射泵注药组(简称T组合组)共计45例,比较两组患儿产前情况、出生情况、使用PS后PaO2/FIO2升高值、双肺不对称复张发生率、肺气漏发生率、2次及以上使用PS率、用氧时间、机械通气率、全身低剂量地塞米松治疗率、1岁以内患呼吸系统疾病次数、1岁以内因呼吸系统疾病住院次数、1岁以内呼吸系统疾病平均住院时间。结果与传统PS用药方式相比,T组合联合注射泵组患儿使用PS后PaO2/FIO2升高更显著,双肺不对称复张率更低、肺气漏发生率更低,机械通气使用率更低,住院过程中用氧时间更短,1岁内呼吸系统疾病住院时间更短。结论与传统方式相比,T组合加压联合注射泵推药方式对于极早产儿NRDS患者是一种更为安全有效的PS用药方式。Objective:To investigate the clinical efficacy of T-combination resuscitator combined with intrapulmonary injection of pulmonary surfactant(PS)using a syringe pump in the treatment of extremely premature infants with neonatal respiratory distress syndrome(NRDS).Method:A total of 60 extremely premature infants diagnosed with NRDS who were admitted to the neonatal intensive care unit of the affiliated hospital of Chuanbei Medical College from January 2018 to September 2020 were selected as the study subjects.The study subjects were treated with tracheal intubation,intratracheal injection of PS,and extubation of CPAP.According to the different methods of positive pressure ventilation and PS injection,they were divided into a resuscitation bag compression combined with syringe injection group(i.e.traditional group)with a total of 60 cases and a T-combination resuscitation device compression combined with injection pump injection group(referred to as T-combination group)with a total of 45 cases.The prenatal situation,birth situation,PaO2/FIO2 increase after PS use,incidence of asymmetric atelectasis,incidence of lung gas leakage,and the use of PS twice or more were compared between the two groups.Oxygen duration,mechanical ventilation rate,systemic low-dose dexamethasone treatment rate,frequency of respiratory system diseases within 1 year old The number of hospitalizations due to respiratory system diseases within the age of 1,and the average length of hospitalization for respiratory system diseases within the age of 1.Compared with the traditional PS medication method,the T combination with injection pump group showed a more significant increase in PaO2/FIO2,lower rate of asymmetric lung recruitment,lower incidence of lung gas leakage,lower use of mechanical ventilation,shorter oxygen use time during hospitalization,and shorter hospitalization time for respiratory diseases within one year old.Conclusion:Compared with traditional methods,the combination of T-group compression and injection pump administration is a sa
