摘　　要：目的探讨基于约束理论的精益化管理在骨科外来医疗器械中的应用效果。方法选取2022年3月至2023年6月医院骨科使用外来器械的手术共560台,根据管理方式不同分为观察组和对照组,每组280台。对照组采用常规管理,观察组采用基于约束理论的精益化管理。比较两组器械的供应质量、管理质量、器械相关缺陷事件情况、术后切口感染率和器械使用满意度。结果观察组器械供应及时率、器械供应正确率和器械供应合格率均高于对照组(P<0.05);观察组器械清洗消毒合格率、器械拆卸合格率、器械组装合格率及器械包装合格率均高于对照组,湿包发生率低于对照组(P<0.05);观察组器械种类或数量不符率、器械丢失率及器械损坏率均低于对照组(P<0.05);观察组术后切口感染率低于对照组(P<0.05);观察组器械使用满意度高于对照组(P<0.05)。结论基于约束理论的精益化管理可提高骨科外来医疗器械的供应质量和管理质量,降低器械相关缺陷事件及切口感染率,提高医护人员使用满意度。Objective To explore the application of lean management based on constraint theory in standardized management of orthopedic outpatient medical devices.Methods A total of 560 surgeries using external devices by the hospital’s orthopedic department from March 2022 to June 2023 were selected and divided into observation group and control group,with 280 surgeries in each group.The control group used conventional management methods,while the observation group implemented lean management based on constraint theory.The quality of external device supply,management,device-related defect incidents,postoperative wound infection rate,and device satisfaction among medical staff were compared between two groups.Results The observation group showed higher rates of timely device supply,correct device supply,and qualified device supply compared with control group(P<0.05).The observation group also had higher rates of device cleaning and disinfection compliance,device disassembly compliance,device assembly compliance,and device packaging compliance,with a lower occurrence of wet packs compared to control group(P<0.05).The observation group had lower rates of device type or quantity discrepancies,device loss,and device damage compared to control group(P<0.05).The postoperative wound infection rate was lower in observation group than that in control group(P<0.05).The device satisfaction among medical staff was higher in observation group than that in control group(P<0.05).Conclusion Lean Management based on constraint theory can to some extent compensate for the shortcomings in the standardized management of orthopedic outpatient medical devices,significantly improving the quality of device supply and management,reducing device-related defect incidents and wound infection rates,and enhancing device satisfaction among medical staff.
