凉膈白虎汤加减联合吸入性乙酰半胱氨酸对重症肺炎患者中医证候积分及血气分析指标的影响

摘　　要：目的分析凉膈白虎汤加减联合吸入性乙酰半胱氨酸对重症肺炎患者中医证候积分及血气分析指标的影响。方法选取2022年5月至2024年5月永春县医院80例重症肺炎患者,根据治疗方法将采用吸入性乙酰半胱氨酸的40例患者纳入对照组,将采用吸入性乙酰半胱氨酸+凉膈白虎汤的40例患者纳入研究组。比较两组临床疗效、中医证候积分、疾病相关评分[急性生理与慢性健康评分Ⅱ(APACHEⅡ)和临床肺部感染评分(CPIS)]、呼吸力学指标[肺动态顺应性(Cdyn)、气道阻力(Raw)和平台压(Pplat)]、血气分析指标[血氧分压(PaO_(2))、血氧饱和度(SaO_(2))和二氧化碳分压(PaCO_(2))]。结果研究组治疗总有效率为92.50%,高于对照组的75.00%(P<0.05)。治疗后,研究组中医证候积分的主证、次证和总积分均低于对照组(P<0.05);研究组APACHEⅡ评分和CPIS评分均低于对照组(P<0.05);研究组Cdyn高于对照组,Raw和Pplat低于对照组(P<0.05);研究组PaCO_(2)低于对照组,SaO_(2)和PaO_(2)高于对照组(P<0.05)。结论凉膈白虎汤加减联合吸入性乙酰半胱氨酸治疗重症肺炎效果显著,有助于减轻患者中医证候,减轻病情,还可调节呼吸力学,减轻肺感染程度,改善患者血气分析指标。Objective To analyze the impact of modified Liangge Baihu Tang combined with inhaled acetylcysteine on the traditional Chinese medicine(TCM)syndrome score and blood gas analysis indices in patients with severe pneumonia.Methods A total of 80 patients with severe pneumonia treated at Yongchun County Hospital from May 2022 to May 2024 was conducted.Forty patients who received inhaled acetylcysteine were included in the control group,and forty patients who received inhaled acetylcysteine plus modified Liangge Baihu Tang were included in the study group.Clinical efficacy,TCM syndrome scores,disease-related scores[Acute Physiology and Chronic Health Evaluation II(APACHEⅡ)and Clinical Pulmonary Infection Score(CPIS)],respiratory mechanics indices[dynamic lung compliance(Cdyn),airway resistance(Raw),and plateau pressure(Pplat)],and blood gas analysis indices[partial pressure of oxygen(PaO_(2)),arterial oxygen saturation(SaO_(2)),and partial pressure of carbon dioxide(PaCO_(2))]were compared between the two groups.Results The total effective rate of treatment in the study group was 92.50%,higher than the 75.00%in the control group(P<0.05).After treatment,the main symptoms,secondary symptoms,and total scores of TCM syndrome in the study group were all lower than those in the control group(P<0.05).After treatment,the APACHEⅡscores and CPIS scores in the study group were both lower than those in the control group(P<0.05).After treatment,Cdyn in the study group was higher,while Raw and Pplat were lower than those in the control group(P<0.05).After treatment,PaCO_(2)in the study group was lower,while SaO_(2)and PaO_(2)were higher than those in the control group(P<0.05).Conclusions The treatment of severe pneumonia with modified Liangge Baihu Tang combined with inhaled acetylcysteine is significantly effective,which helps to alleviate the TCM syndrome of patients,reduce the severity of the disease,regulate respiratory mechanics,reduce the extent of lung infection,and improve the blood gas analysis indices of patients.
