大疱性类天疱疮合并下肢深静脉血栓形成1例

摘　　要：大疱性类天疱疮(Bullous Pemphigoid,BP)作为自身免疫性疾病的一种,好发于老年人,其疱壁较厚,疱液清,不易破溃,组织病理为表皮下水疱,常伴基底膜带免疫球蛋白(Immunoglobulin,Ig)G和(或)补体C3沉积。该病病程长,严重者可泛发全身,极易引起机体消耗性衰竭,西医常以糖皮质激素作为治疗本病的首选药物,但激素的长期使用易引起多种并发症甚至多脏器衰竭,临床上如何帮助患者改善症状及预后成为皮肤科医生的一大难题。山东中医药大学附属医院于2023年3月收治BP合并下肢深静脉血栓形成(Deep Vein Thrombosis,DVT)患者1例,经中西医结合诊治,患者皮损消退,DVT逐渐稳定。文章报道了该患者的临床诊治过程,并借此探讨BP与DVT之间的潜在关联及预防措施,以期为临床提供参考。Bullous pemphigoid,as a type of autoimmune disease,is more common in elderly individuals.It is characterized by thick-walled blisters with clear fluid that are resistant to rupture.The histopathology shows subepidermal blisters,often accompanied by deposits of IgG and/or C3 along the basement membrane.The disease has a prolonged course,and can involve the entire body of severe cases,leading to systemic exhaustion.In Western medicine,glucocorticoids are commonly used as the first-choice treatment for this condition.However,prolonged use of steroids can lead to various complications,even multiple organ failure.How to relieve symptoms and improve prognosis in clinical practice has become a major problem for dermatologists.In March 2023,the Affiliated Hospital of Shandong University of Traditional Chinese Medicine admitted one case of bullous pemphigoid plus lower extremity deep vein thrombosis.After treatment in the integrative medicine,the skin lesions of the patient subsided and the lower extremity deep vein thrombosis gradually stabilized.This article reports the clinical diagnosis and treatment process of this patient,and discusses the potential association and preventive measures between bullous pemphigoid and lower extremity deep vein thrombosis,aiming to provide clinical reference.
