摘　　要：多囊卵巢综合征(Polycystic Ovary Syndrome,PCOS)是育龄期妇女常见的内分泌疾病之一,其中约有50%的患者伴有超重或肥胖。西医对此病的发病机制暂未明确,临床以长期对症治疗为主,但按症状其可归属于中医“月经后期”“闭经”“癥瘕”“不孕”等范畴。陈林兴教授系云南省名中医,从事中医妇科临床与教学工作37年余,擅长治疗PCOS、月经不调、不孕等多种妇科常见、疑难疾病。陈林兴教授认为脾肾两虚是PCOS的主要病机,其中肥胖型PCOS多以脾肾两虚为本、痰湿蕴结为标,治疗时应以补肾健脾、燥湿化痰为基本原则,以期达到帮助患者减重,促进患者卵泡发育、排出及月经正常来潮的目的。导师陈林兴教授运用自拟经验方“补肾化痰方”治疗肥胖型PCOS临床疗效显著,全方以六味地黄汤、四物汤、二陈汤为基础,配以菟丝子补益肾气,党参、炙黄芪健脾益气,胆南星清热化痰降脂,并根据患者就诊时症状、所属月经周期及辅助检查结果加减用药,做到个体化对症治疗的同时重视指导患者改善生活方式、调节情志。现将导师陈林兴教授治疗肥胖型PCOS的思想及经验整理介绍如下,并附典型医案1例,以期为临床治疗肥胖型PCOS提供新思路。Polycystic ovary syndrome(PCOS)is one of the most common endocrine diseases in women of childbearing age,and more than half of them are overweight or obese.Western medicine is not clear about the pathogenesis of this disease,long-term clinical treatment is mainly symptomatic,but according to its symptoms,traditional Chinese medicine classifies it into late menstruation,amenorrhea,mass,infertility and other categories.Professor CHEN Linxing is a famous traditional Chinese medicine practitioner in Yunnan Province,has been engaged in the clinical and teaching work of TCM gynecology for more than 37 years,and is good at treating many common and difficult diseases of gynecology such as polycystic ovary syndrome,irregular menstruation and infertility.Professor CHEN Linxing believes that spleen and kidney deficiency is the main pathogenesis of polycystic ovary syndrome,of which obesity type polycystic ovary syndrome is mainly based on spleen and kidney deficiency,phlegm and dampness accumulation are the symptoms,the treatment should be based on the basic principles of tonifying kidney and spleen,and drying dampness and eliminating phlegm,in order to achieve the goal of helping patients lose weight,promoting the development and discharge of follicles,and normal menstruation.Professor CHEN Linxing,the mentor,has achieved significant clinical efficacy in treating obese patients with polycystic ovary syndrome using self-formulated prescription,the Bushen Huatan prescription(补肾化痰方).The prescription is based on the classic prescriptions of Liuwei Dihuang decoction(六味地黄汤),Siwu decoction(四物汤)and Erchen decoction(二陈汤),in which Tusizi(Semen Cuscutae)is added to reinforce kidney Qi(气),Dangshen(Codonopsis)and Zhihuangqi(Radix astragalus)are used to strengthen spleen and promote Qi,and Dannanxing(arisaema cum bile)is used to clear heat,dispel phlegm and lower blood lipids,and adjustments are made according to the patient’s symptoms,phase of the menstrual cycle and the results of auxiliary examinat
