基于“一身之力出于带脉”理论探讨痿证论治

摘　　要：近年来痿证患者越来越多,中医药治疗痿证具有明显优势,其中“治痿独取阳明”为治疗痿证的代表方法。本研究主要探讨名老中医蔡圣朝教授从带脉论治痿证的经验,为临床提供新的思路与方法。《杂病源流犀烛》:“一身之强力亦赖带脉以出。”力出于带脉,无力而成痿,故带脉与痿证联系密切。带脉为全身唯一的横向经络,为奇经中的奇脉,能通行元气,为人体基本生命活动提供动力,通补带脉能够补充元气。从西医上说,由于环腰循行的特殊生理位置,带脉系核心肌群,可保持人体运动平衡。带脉又为脾脉,易受湿邪困阻,带脉根于肾、本于脾,赖先天精气与后天水谷精微的供养,督脉络于带脉,补充带脉阳气,阳明脉充养带脉经气气血,从而维持肢体关节的活动,若带脉受损,元气无法周流全身,核心肌群失用,则周身肌肉无力,发为痿证。蔡圣朝教授论治痿证时常采用针药结合法通调带脉,临床效果显著且患者易于接受。在论治痿证时以补带脉为思路,运用祛湿健脾、补肾健脾、调阳明脉、温督补阳等方法使带脉通畅而充实,以促进痿证患者康复。In recent years,there are more and more patients with flaccidity syndrome.Traditional Chinese medicine shows obvious Iadvantages in the treatment of flaccidity syndrome,among which the method of treating flaccidity syndrome based on the theory of“choosing acupoints on stomach channel to treat flaccidity syndrome”is a representative treatment method.In this study,the experience of Professor CAI Shengzhao,a famous veteran TCM practitioner,in treating flaccidity syndrome is explored to provide new ideas and methods for medical practitioners.Zhabing Yuanliu Xizhu(《杂病源流犀烛》)records that the strength of the whole body originates from belt channel.The strength originates from belt channel,and the weakness means flaccidity syndrome,so the belt channel is closely related to flaccidity syndrome.Belt channel is the only transverse meridian of the whole body,and the odd channel of eight extraordinary meridians.Belt channel can pass vitality and provide power for the basic life activities of the human body in TCM.Tonifying belt channel can replenish vitality.In Western medicine,due to the special physiological position of the circumferential lumbar circulation,the belt vessel is the core muscle group and can maintains the balance of the human body movement.The belt channel is also the spleen channel,which is easily hindered by dampness.The belt channel is rooted in kidneys and spleen,relying on the supply of innate essence and acquired water and grains.Du Vessel(督脉)is connected to the belt channel,supplementing Yangqi(阳气)for the belt vessel,and Yangming Vessel(阳明脉)nourishes Qi(气)and blood for the belt vessel to maintain the activities of the joints and the limbs.When the belt vessel is damaged,the vital energy can not flow throughout the whole body,the core muscles group is useless,then the muscles around the body are weak,resulting in flaccidity syndrome.Prof.CAI Shengchao often uses acupuncture plus Chinese medicines to treat flaccidity syndrome by regulating the belt channel,and has gaine
