摘 要:足细胞作为肾小球滤过屏障的重要组成部分,其损伤与糖尿病肾病的发生、发展有紧密联系。一系列涉及足细胞损伤的关键信号通路,如磷脂酰肌醇3激酶/蛋白激酶B、核转录因子-κB、糖原合成酶激酶-3β、转化生长因子-β1及Wnt/β-连环蛋白等,在糖尿病肾病中扮演着重要角色,且在糖尿病肾病中的具体作用机制各不相同,它们的异常可能导致肾小球滤过屏障被破坏,进一步促进糖尿病肾病进展。中药干预糖尿病肾病的研究表明,中药单体和复方能够通过多种途径干预这些信号通路,从而治疗足细胞损伤。中药的作用机制复杂多样,它们可以直接抑制或激活特定的信号通路,也可以通过调节氧化应激、炎症反应等间接影响足细胞损伤。中药干预糖尿病肾病的优势在于多靶点、多通路的治疗方式,不仅能更有效地减轻足细胞损伤,也能够减少药物的不良反应和耐药性。可见中药在干预糖尿病肾病足细胞损伤方面具有广阔的应用前景。Podocyte is an important part of glomerular filtration barrier,and its injury is closely related to the occurrence and development of diabetic nephropathy.A series of key signaling pathways involved in podocellular injury,such as PI3K/Akt,NF-κB,GSK-3β,TGF-β1,and Wnt/β-catenin,played an important role in diabetic nephropathy,and the specific action mechanism of these signaling pathways in diabetic nephropathy is different.Their abnormalities may lead to the breakdown of the glomerular filtration barrier,further promoting the progression of diabetic nephropathy.In the study of TCM medicine intervention on diabetic nephropathy,it is found that TCM medicine monomer and compound can interfere with these signaling pathways through various ways,so as to treat podocyte injury.The action mechanism of TCM medicine is complex and diverse.They can directly inhibit or activate specific signaling pathways,or indirectly prevent podocyte damage by regulating oxidative stress,inflammation response and so on.The advantage of TCM medicine intervention on diabetic nephropathy lies in its multi-target and multi-pathway treatment characteristics.This multi-target,multi-pathway treatment can not only reduce the damage of podocytes more effectively,but also reduce the adverse reactions and drug resistance.Therefore,TCM medicine has a broad application prospect in the intervention on podocyte injury in diabetic nephropathy.
