中药联合抗病毒药物治疗单纯疱疹病毒性角膜炎的疗效和安全性:系统评价与Meta分析

摘　　要：目的:运用Meta分析对中药联合抗病毒药物治疗单纯疱疹病毒性角膜炎(Herpes Simplex Keratitis,HSK)的有效性和安全性进行系统评价。方法:对知网、万方、维普、中国生物医学文献服务系统、PubMed、Embase、Cochrane Library、Web of Science 8大数据库进行全面且系统的检索,筛选建库至2023年12月国内外关于中药和抗病毒药物联合治疗HSK的随机对照试验文献,通过软件与人工筛选去重后阅读摘要及全文,根据纳入与排除标准纳入符合条件的文献,对所纳入的研究使用Cochrane偏倚风险评估工具进行质量评价,并运用RevMan 5.4和Stata 14.0软件进行Meta分析。结果:依据纳入与排除标准筛选后最终纳入17个随机对照试验研究,全部为中文文献,涉及患者共1435例。Meta分析结果提示,中药联合抗病毒药物治疗HSK效果显著,可提高总有效率,降低复发率及不良反应发生率,改善中央角膜厚度。结论:中药联合抗病毒药物治疗HSK能够提高疗效,减少复发,缩短疗程,改善患者的中央角膜厚度,且安全性较高。Objective:To systematically evaluate the efficacy and safety of Chinese medicines plus antiviral drugs in the treatment of herpes simplex keratitis(HSK)by a Meta-analysis.Methods:In CNKI,Wanfang,VIP,SinoMed,PubMed,Embase,Cochrane Library,and Web of Science,a comprehensive and systematic search was conducted to screen domestic and international studies on randomized controlled clinical trials of treating HSK with Chinese medicines plus antiviral drugs from inception to December 2023.Duplicative studies were removed through software and manual screening,then abstracts and the full texts were read,and studies that met the criteria were included according to the exclusion and inclusion criteria.The included studies were evaluated for quality assessment by Cochrane bias risk assessment tool,and a Meta-analysis was performed by RevMan 5.4 and Stata 14.0 software.Results:After screening according to the exclusion and inclusion criteria,17 randomized controlled trial studies were finally included,all in Chinese,with 1435 patients in all.The results of Meta-analysis showed that Chinese medicines plus antiviral drugs on HSK showed significant effects,could increase the total efficiency,reduce the recurrence rate and the incidence of adverse reactions,and improve the central corneal thickness of patients.Conclusion:In treating HSK,Chinese medicines plus antiviral drugs can improve the curative effect,reduce the recurrence,shorten the course of treatment,and improve the central corneal thickness of patients,with high safety.
