清热解毒药治疗恶性肿瘤研究进展

摘　　要：热毒,也被称为火毒或温毒,《黄帝内经太素·十二水》指出:“火毒入身,诸骨枯槁,经脉溃脓,名为恶火之病。”体现了热毒致病之酷烈。现代中医肿瘤学的癌毒病机理论指出,癌毒多由化热、酿痰、生瘀、致虚等一系列病机演变形成,其中热毒蕴结是引起恶性肿瘤形成与扩散的病机之一。同时肿瘤的机械压迫导致脏器管腔及血脉受压或梗阻,引起的炎症或感染又成为促使肿瘤恶化和进展的因素之一。清热解毒药多苦寒,可缓解各种热毒及火毒郁结所致的临床证候,减轻炎症,且具有中医药多靶点、价格低廉、研究前景广阔等优点,贯穿恶性肿瘤防治的全过程。因此,清热解毒药物在预防和治疗恶性肿瘤方面具有独特的优势。本研究总结、分析近年清热解毒药治疗恶性肿瘤的理论、临床和实验研究等方面的文献,发现其主要通过调控信号通路抑制肿瘤细胞增殖、诱导细胞凋亡、增强免疫功能、改善肿瘤炎性微环境、逆转细胞多药耐药、抑制肿瘤血管生成、减轻化疗后不良反应、缓解癌痛等,发挥抗癌作用,但同时有消化系统不良反应、使用不当引起的神经毒性等风险。本研究对相关文献进行总结与分析,概述清热解毒药物在预防和治疗恶性肿瘤方面发挥的积极作用及相关风险,为临床诊疗提供一定的依据与参考。Heat poison is also known as fire poison or warm poison.Huangdi Neijing Taisu(《黄帝内经太素》):After fire Hpoison enters into the body,bones become withered,meridians are broken and purulent,called the disease of evil fire,which embodies the fierce of heat toxin disease.The pathogenesis theory of cancer toxicity in modern TCM oncology emphasizes that cancer toxicity is mainly formed by a series of pathogenesis evolution of heat,phlegm,stasis and deficiency.The accumulation of heat toxicity is one of the pathogenesis that causes the formation and spread of malignant tumors.At the same time,the mechanical compression of the tumor leads to the pressure or obstruction of the organ lumen and blood vessels,and the inflammation or infection caused by it is also one of the factors that promote the deterioration and development of the tumor.Heat-clearing and detoxifying Chinese medicine is bitter and cold,can relieve the clinical symptoms caused by various heat toxicity and fire toxicity stasis,and reduce inflammation.Besides,it has the advantages of multiple targets,low price and broad research prospect of traditional Chinese medicine,which runs through the whole process of malignant tumor prevention and treatment.Therefore,heat-clearing and detoxification drugs have unique advantages in the prevention and treatment of malignant tumors.Recent theoretical,clinical and experimental studies on the treatment of malignant tumors with heat-clearing detoxifying Chinese medicines are summarized and analyzed,and it is found that it mainly plays an anticancer role by regulating signal pathways,inhibiting tumor cell proliferation,inducing cell apoptosis,enhancing immune function,improving tumor inflammatory microenvironment,reversing cell multi-drug resistance,inhibiting tumor angiogenesis,alleviating adverse reactions after chemotherapy,relieving cancer pain,etc..At the same time,there are risks of adverse reactions to the digestive system and neurotoxicity caused by improper use.In summary,through the summary and analysi
