从命门-肾气-天癸学说探讨特发性性早熟的病证特点

摘　　要：命门-肾气-天癸学说是以命门为核心的中医学理论,体现了中医的整体观念和阴阳调和的思想。特发性性早熟的病机关键为肾的阴阳失调,症状则表现为全身的阴阳失调。现代西医研究证实中枢性性早熟的发病机制为下丘脑-垂体-性腺轴功能失调所致。笔者认为命门-肾气-天癸体系作为以肾藏象为核心的理论,涵盖了下丘脑-垂体-性腺轴的功能,命门-肾气-天癸的功能失调在特发性性早熟的各个阶段均有体现。该文尝试从特发性性早熟的发病机制、命门-肾气-天癸与下丘脑-垂体-性腺轴的关系出发,探讨特发性性早熟的不同时期的病证特点以及对于临床治疗的启发。The theory of Mingmen-kidney Qi-Tiangui is a traditional Chinese medicine theory with Mingmen as the core,reflecting the overall concept of traditional Chinese medicine and the idea of harmony between Yin and Yang.The key to the pathogenesis of idiopathic precocious puberty is Yin and Yang imbalance of kidney,and the symptoms are Yin and Yang disorders of the whole body.Modern Western medicine studies have confirmed that the pathogenesis of central precocious puberty is caused by hypothalamus-pituitary-gonad axis dysfunction.The author believes that the theory of Mingmen-kidney Qi-Tiangui sys⁃tem covers the function of the hypothalamus-pituitary-gonad axis.As a theoretical system,it is the core of theory of organ pic⁃ture of kidney.The disorders of Mingmen-kidney Qi-Tiangui is reflected in all stages of idiopathic precocious puberty.This article tried to explore the characteristics of different periods of idiopathic precocious puberty and the inspiration for clinical treat⁃ment which was based on the relationship between the pathogenesis of idiopathic precocious puberty,Mingmen-kidney Qi-Tian⁃gui and hypothalamus-pituitary-gonad axis.
